摘 要:目的应用光学相干断层扫描血管成像(OCTA)检测孔源性视网膜脱离(RRD)患者行玻璃体腔硅油取出前后黄斑中心凹无血管区面积的变化及黄斑区血流密度变化。方法回顾性分析2019年1月~2019年6月在潍坊医学院附属医院眼科中心首诊为单眼RRD并且行视网膜脱离复位术(首次行玻璃体切割+视网膜激光光凝+玻璃体腔硅油填充术,3个月行玻璃体腔硅油取出+白内障超声乳化吸出联合人工晶体植入术)患者20例(20眼),设计3组:RRD患眼取油前组(A组)、RRD患眼取油后1周组(B组)和对照组(C组,患眼的对侧眼)。所有患者行眼科常规检查及眼轴测量、OCTA检查,OCTA主要检测指标包括:浅层黄斑区中心凹无血管区(FAZ)面积、黄斑区旁中心凹处浅层视网膜毛细血管丛(SCP)血流密度、深层视网膜毛细血管丛(DCP)血流密度值。分别比较3组之间的FAZ面积、SCP血流密度、DCP血流密度的变化。结果 B组患者浅层FAZ面积较A组减小,差异有显著性(P=0.000),较C组差异无显著性(P>0.05),A组浅层FAZ面积较C组差异有显著性(P=0.003)。B组患者浅层FAZ面积与DCP血流密度呈正相关(r=0.546,P=0.013),与SCP无明显相关性(P>0.05)。B组SCP血流密度(P=0.006)、DCP血流密度(P=0.032)较C组均降低,差异有显著性,与A组间差异无显著性。结论 RRD患者玻璃体腔硅油填充状态可能会使浅层FAZ面积扩大,孔源性视网膜脱离复位术后黄斑区旁中心凹血流短期内有显著改变。Objective To investigate the foveal avascular zone size and macular blood flow density at parafoveal area changes before and after silicone oil removal in patient with rhegmatogenous retinal detachment(RRD) with optical coherence tomography angiography(OCTA).Methods In this retrospective study,20 eyes in 20 patients with RRD for first time underwent reattachment of retinal detachment(First time they underwent pars plana vitrectomy,laser coagulation and silicone oil tamponade,and the second operation for silicone oil removal and phacoemulsification combined with intraocular lens implantation) in the Eye Center of the Affiliated Hospital of Weifang Medical University from January 2019 to June 2019.There were three groups:the group before silicon oil removal with RRD(group A),the group 1 w after silicon oil removal with RRD(group B),and the control group(group C,the opposite eye of the affected eye).All patients underwent binocular axial measurement and OCTA.The comparative observation of the changes of the superficial foveal avascular zone size before and after the silicone oil removal and the changes of blood flow density of the superficial capillary plexus and deep capillary plexus at parafoveal area were made.Results In group B,the superficial FAZ size was smaller than group A,and the difference was statistically significant(P=0.000),and there was no significant difference compared with group C(P>0.05).The superficial FAZ size group A was significantly different from that in group C(P=0.003).The superficial FAZ size in group B was positively correlated with DCP blood flow density at parafoveal area(r=0.546,P=0.013),and had no significant correlation with SCP(P>0.05).In group B,the SCP blood flow density at parafoveal area was lower than that in group C,and the difference was statistically significant(P=0.006),the same as DCP blood flow density at parafoveal area(P=0.032),and there was no significant difference compared with group A.Conclusion In patients with RRD,the silicone oil tamponade may enlarge superficia
