肌壁间子宫肌瘤宫腔镜切除术的可行性及患者生殖预后研究

摘　　要：目的:分析肌壁间子宫肌瘤宫腔镜切除术的可行性及患者生殖预后情况。方法:选择2018年6月~2020年6月在本院治疗的肌壁间子宫肌瘤患者58例,所有患者皆采用宫腔镜切除术治疗,观察患者的症状缓解情况,术后肌层恢复厚度变化,子宫肌瘤大小,药物应用情况,同时注意患者的手术指标。结果:手术时间(52.05±18.32)min,术中出血量(53.35±10.62)mL,手术前后瘤窝至子宫肌层厚度依次为(3.25±1.11)mm、(8.28±1.36)mm,数据有统计学意义(P<0.05)。两组患者术中均未出现严重并发症,53例患者行一期手术切除,子宫肌瘤(4.25±1.11)cm,5例通过二期手术切除,肌瘤大小(4.37±1.16)cm,一、二期手术子宫肌瘤大小差异不明显(P>0.05)。结论:3型子宫肌瘤宫腔镜切除术的可行性较高,并且患者生殖预后情况较理想。Objective:To analyze the feasibility of hysterectomy for intramural uterine fibroids and the reproductive prognosis of the patients.Methods:58 patients with intramural uterine fibroids treated in our hospital from June 2018 to June 2020 were selected,all of whom were treated by hysteroscopic resection.Symptom relief,changes in postoperative myometrium thickness,uterine fibroids size,and drug application were observed,and surgical indicators of the patients were also noted.Results:The operative time was(52.05±18.32)min,the intraoperative blood loss was(53.35±10.62)mL,and the thickness from the fossa to myometrium before and after the operation was(3.25±1.11)mm and(8.28±1.36)mm,respectively,with statistical significance(P<0.05).There were no serious complications in the two groups.53 patients underwent one-stage operation,and the uterine fibroids were(4.25±1.11)cm,and 5 cases were resected by secondary surgery.The size of myoma was(4.37±1.16)cm.There was no significant difference in the size of hysteromyoma between the first and second stage operation(P>0.05).Conclusion:Hysterectomy for type 3 hysteromyoma is feasible and the reproductive prognosis of the patients is ideal.
