摘  要:目的探讨急性脑梗死(ACI)患者外周血微RNAs(miRNAs)表达情况及其与炎性因子间的相关性。方法选取2018年6月至2019年8月该院收治的首次发病的ACI患者316例为ACI组,另选取同期体检的健康志愿者60例为对照组。采用定量逆转录PCR(qRT-PCR)测定各组血浆miRNA-124、miRNA-128b、miRNA-153、miRNA-424,采用ELISA测定血清炎性因子[白细胞介素(IL)-6、IL-8、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)]。结果ACI组血浆miRNA-124、miRNA-424水平明显低于对照组,血浆miRNA-128b、miRNA-153及血清IL-6、IL-8、TNF-α、CRP水平明显高于对照组(P<0.05)。中、重度组ACI患者血浆miRNA-128b、miRNA-153及血清IL-6、IL-8、TNF-α、CRP水平明显高于轻度组,且重度组明显高于中度组(P<0.05);中、重度组ACI患者血浆miRNA-124、miRNA-424水平明显低于轻度组,且重度组明显低于轻度组(P<0.05)。大、中梗死灶组ACI患者血浆miRNA-128b、miRNA-153及血清IL-6、TNF-α、CRP水平高于小梗死灶组,且大梗死灶组高于中梗死灶组(P<0.05);大、中梗死灶组ACI患者血浆miRNA-124、miRNA-424表达水平明显低于小梗死灶组,且大梗死灶组明显低于中梗死灶组(P<0.05)。ACI患者血浆miRNA-124、miRNA-424表达水平与血清IL-6、IL-8、TNF-α、CRP水平呈负相关,miRNA-128b、miRNA-153表达水平与血清IL-6、IL-8、TNF-α、CRP水平呈正相关(P<0.05)。结论ACI患者循环miRNAs存在差异表达,miRNA-124、miRNA-424呈低表达,而miRNA-128b、miRNA-153呈高表达,且与炎性因子水平密切相关,可作为ACI病情评估的潜在生物标志物。Objective To investigate the expression of microRNAs(miRNAs)in peripheral blood of patients with acute cerebral infarction(ACI)and their correlation with inflammatory factors.Methods A total of 316 first-onset ACI patients admitted to this hospital from June 2018 to August 2019 were selected as the ACI group,and 60 healthy volunteers who received physical examination during the same period were selected as the control group.The levels of plasma miRNA-124,miRNA-128b,miRNA-153 and miRNA-424 were detected by quantitative reverse transcription PCR(qRT-PCR),and the serum levels of inflammatory factors,including interleukin(IL)-6,IL-8,tumor necrosis factor-alpha(TNF-α)and C-reactive protein(CRP),were measured by ELISA.Results The levels of plasma miRNA-124 and miRNA-424 in the ACI group were lower than those in the control group,and the levels of plasma miRNA-128b and miRNA-153,serum IL-6,IL-8,TNF-αand CRP were higher than those in the control group(P<0.05).The levels of plasma miRNA-128b and miRNA-153,and serum IL-6,IL-8,TNF-αand CRP in the moderate and severe ACI groups were significantly higher than those in the mild ACI group,and the levels of above indicators in the severe group were higher than those in the moderate group(P<0.05).The levels of plasma miRNA-124 and miRNA-424 in the moderate and severe ACI groups were significantly lower than those in the mild ACI group,and the levels of above indicators in the severe group were lower than those in the moderate group(P<0.05).The levels of plasma miRNA-128b and miRNA-153,and serum IL-6,TNF-αand CRP in the large and medium infarct size groups were significantly higher than those in the small infarct size group,and the levels of above indicators in the large infarct size group were higher than those in the medium infarct size group(P<0.05).The levels of plasma miRNA-124 and miRNA-424 in the large and medium infarct size groups were significantly lower than those in the small infarct size group,and the levels of above indicators in the large infarct size group were
