摘　　要：目的探讨对比增强能谱乳腺X线摄影(CESM)诊断乳腺癌的价值。方法回顾性分析经乳腺X线摄影(MG)筛查BI-RADS分类为4A类及以上病例进行乳腺CESM及MRI检查,并且最终经病理证实的50例52个病灶。所有患者均行双侧乳腺CESM和MRI检查。对病理标本进行判读,获得病理分类诊断结果,观察CESM和MRI表现,记录病灶检出情况,并对病灶进行定性诊断。以病理结果为金标准,采用卡方检验分析CESM及MRI的诊断效能。结果50例患者病理共检出52个病灶,良性病灶13个,恶性病灶39个;CESM检出病灶38个,MRI检出病灶34个,定性诊断判读错误病灶CESM 1个,MRI 5个。以病理结果为金标准,CESM诊断乳腺癌的敏感性、特异性和正确指数分别为97.40%、92.3%和0.897,MRI分别为87.2%、84.6%和0.718,差异均无统计学意义。结论CESM诊断乳腺癌与MRI检查的诊断效能接近。Objective To explore the value of contrast-enhanced energy spectrum mammography(CESM)in the diagnosis of breast cancer.Methods A retrospective analysis was performed on the cases classified as 4 A and above by BI-RADS by mammography-MG screening,and 50 cases with 52 lesions were finally confirmed by pathology.All patients underwent bilateral CESM and MRI examinations.Pathological specimens were interpreted to obtain the results of pathological classification,CESM and MRI were observed,the detection of lesions was recorded,and the qualitative diagnosis of lesions was made.The diagnostic efficacy of CESM and MRI was analyzed by chi-square test based on pathological results.Results There were 52 lesions,13 benign lesions and 39 malignant lesions in 50 patients.Thirty-eight lesions were detected by CESM and Thirty-four lesions were detected by MRI,and one of CESM and five of MRI was misinterpreted by qualitative diagnosis.With the pathological results as the gold standard,The sensitivity,specificity and accuracy indices of CESM in the diagnosis of breast cancer were 97.40%,92.3%and 0.897 respectively and MRI was 87.2%,84.6%and 0.718,respectively,There was no statistically significant difference between the two groups.Conclusion The diagnostic efficacy of CESM in breast cancer is similar to that of MRI.
