冠心病合并糖尿病中医证候分布规律研究

摘　　要：目的研究冠心病合并糖尿病患者中医证候分布规律。方法收集2016年10月至2018年03月期间符合冠心病及2型糖尿病诊断标准的患者347例,统计其中医四诊信息,保留出现频率≥25%的条目,采用因子分析中的主成分分析,提取公因子,结合专业知识及临床经验对合并后公因子进行证候命名和解释。结果因子分析共提取11个公因子,结合专业知识及临床经验合并后得出冠心病合并糖尿病的病性主要为痰湿、血瘀、阴虚、火热、气虚,病位在心、脾、肝、肾。将公因子根据证候特征重新组合,归纳出该病的中医证型为气虚血瘀、气阴两虚、痰浊内阻。结论冠心病合并糖尿病的中医四诊信息分布呈现一定特征规律,其病性以痰、瘀、虚为主,病位在心、脾、肝、肾,主要证型为气虚血瘀、气阴两虚、痰浊内阻。Objective To observe the distribution characteristics of TCM syndrome and four diagnostic methods of patients with coronary heart disease complicated with diabetes mellitus. Methods Information from four diagnostic methods of 347 patients with coronary heart disease complicated with diabetes mellitus were collected;and information with the occurrence rate of 25% or above was retained and factorial analysis was used for statistic analysis and extracted common factors. Combined with professional knowledge and clinical experience, syndrome naming and interpretation of combined common factors were conducted.Results Factor analysis found 11 common factors. The common factors showed the character of TCM syndrome elements were phlegm-damp, blood stasis, Yin deficiency, heat syndrome, Qi deficiency, etc. The location of TCM syndromes elements were heart,spleen,liver and kidney in coronary heart disease complicated with diabetes mellitus patients. Qi deficiency and blood stasis, deficiency of Qi and Yin and obstruction of phlegm were the most possible TCM syndrome types through the distribution and combination characteristics of coronary heart disease complicated with diabetes mellitus TCM syndrome elements. Conclusion There exists a certain rule of four diagnostic methods of coronary heart disease complicated with diabetes mellitus patients. The phlegm, blood stasis and deficiency are characterized as the main characters of TCM syndrome elements, and heart, spleen, liver and kidney are characterized as the main location of TCM syndrome elements. The main syndromes are Qi deficiency and blood stasis, Qi and Yin deficiency, phlegm obstruction.
