萎缩性胃炎伴上皮内瘤变人群性别、年龄、病理、部位分布等特点的探讨

摘　　要：[目的]探讨萎缩性胃炎伴上皮内瘤变人群的性别、年龄、病理、部位分布等特点。[方法]入选门诊胃镜及体检中心胃镜检查的萎缩性胃炎伴上皮内瘤变患者480例,将患者一般信息、病理结果、分布部位记录于设计好的记录表中,将搜集到的信息进行录入、整理、统计学分析,得出不同性别、年龄、病理分型和发病部位分布特点。[结果]480例患者中男332例(69.17%),女148例(30.83%),男性比率大于女性。年龄38~94(平均61.08)岁,男性平均年龄60.81岁,女性平均年龄61.68岁;≤45岁36例,>45~≤55岁92例,>55~≤65岁224例,>65岁128例。低级别上皮内瘤变400例(83.33%),男性288例(16.67%),女性112例(23.33%);高级别上皮内瘤变80例(16.67%),男性44例(9.17%),女性36例(7.50%)。上皮内瘤变发生在胃窦232例(48.33%),胃角172例(35.83%),胃体40例(8.34%),胃底36例(7.50%)。[结论]萎缩性胃炎伴上皮内瘤变人群中,男性发病率较女性高,发病年龄段以>55~≤65岁多见;病理诊断为低级别上皮内瘤变的发病人数较高级别上皮内瘤变人数多;发生部位以胃窦部较多见,其次为胃角、胃体。[Objective]To investigate the gender,age,pathology,site distribution in patients of atrophic gastritis with intraepithelial neoplasia.[Methods]Total 480 cases urdergoing gastroscopy examination in the clinic or physical examination center were collected.The patients’general information,pathological results and lesions distribution areas were recorded in a well-designed record table.The collected information were input,sorted,statistically analyzed,and the correlation between different pathological types and TCM constitution were compared.The distribution characteristics of different sex,age,pathological type and site were recorded.[Results]Among the 480 cases of atrophic gastritis with intraepithelial neoplasia,there were more male cases(332,69.17%)than female cases(148,30.83%).The eldest patient was 94 years old,the youngest was 38 years old.The overall average age was 61.08 years.The average age of the male was 60.81 years old,and the average age of the female was 61.68 years.The number of patients aged≤45 years was 36,aged 45 to 55 years was 92,aged 55 to 65 years was 224,aged>65 years was 128,there was significant difference in the incidence of atrophic gastritis with intraepithelial neoplasia among different age groups,in which incidence occured more frequently in the elder patients.Histopathological classification of"intraepithelial neoplasia":low grade intraepithelial neoplasia was diagnosed in 400 cases(83.33%),in which male was 288 cases(16.67%),female was 112 cases(23.33%).In the high-grade intraepithelial neoplasia(16.67%),44 cases(9.17%)were male,and 36 cases(7.50%)were female.The incidence of low grade intraepithelial neoplasia was significantly higher than high grade intraepithelial neoplasia.Pathological classification and lesion position:lesion of 232 cases(48.33%)located in gastric antrum,172 cases(35.83%)was in gastric angle,40 cases was in gastric body(8.34%),36 cases were in gastric fundus(7.50%).The incidence of lesion location in atrophic gastritis with intraepithelial neoplasia was diff
