摘　　要：[目的]探讨骨桥蛋白(OPN)和基质金属蛋白酶(MMP)-3、MMP-10在结肠癌中表达情况,旨在为改善该类患者的诊断现状提供一定的理论依据。[方法]收集我院病理科存档结肠癌手术标本100例为观察组,另选取100例健康志愿者的正常肠黏膜标本为对照组。进行OPN、MMP-3、MMP-10蛋白检测后,比较2组OPN、MMP-3、MMP-10阳性表达率,探讨OPN、MMP-3、MMP-10阳性表达与结肠癌临床病例参数的关系,并对OPN、MMP-3、MMP-10检测诊断结肠癌的效能进行分析。[结果]观察组OPN、MMP-3、MMP-10阳性表达率分别为75.00%、78.00%、59.00%,明显高于对照组(8.00%、11.00%、10.00%),差异有统计学意义(P<0.05);肿瘤>5 cm,TNM分期为Ⅲ、Ⅳ期,以及淋巴结转移的患者具有更高的OPN、MMP-3、MMP-10阳性表达率,浆膜浸润的患者具有更高的MMP-3、MMP-10阳性表达率。OPN、MMP-3、MMP-10对结肠癌均有较高的诊断价值,ROC-AUC分别为0.784、0.758、0.807。三指标联合应用则诊断价值更高:灵敏度和特异度分别为0.850(85/100)、0.840(84/100)。[结论]结肠癌组织的OPN、MMP-3、MMP-10阳性表达率明显较高,3种检测因子均可用于结肠癌诊断。[Objective]To study the Osteopontin(OPN)and Matrix Metalloproteinase(MMP)-3,MMP-10 expression in colorectal cancer,aiming to improve the present diagnosis of these patients.[Method]One hundred cases of colon cancer surgical specimens from January 2015 to June 2018 in our department of pathology were selected as the observation group,and the normal intestinal mucosa of 100 healthy volunteers were selected as the control group.The positive expression rates of OPN,MMP-3 and MMP-10 in the two groups after the detection were compared,and the relationship between the positive expression of OPN,MMP-3 and MMP-10 and the clinical case of colon cancer was discussed.The efficacy of OPN,MMP-3,and MMP-10 in the detection among colon cancer was compared.[Results]The positive expression rates of OPN,MMP-3,and MMP-10 in the observation group were 75.00%,78.00%,and 59.00%,respectively,which were significantly higher than those in the control group(8.00%,11.00%,and 10.00%)(P<0.05).Patients with tumors>5 cm,TNM stage III,IV,and lymph node metastasis had a higher rate of positive expression of OPN,MMP-3,MMP-10,patients with plasma membrane infiltration had higher MMP-3,MMP-10 positive expression rate.The OPN MMP-3 MMP-10 had higher diagnostic value for colon cancer,and the ROC-AUC were 0.784,0.758,0.807,respectively.The combined application of the three indicators had a higher diagnostic value:the sensitivity and specificity were 0.850(85/100),0.840(84/100)respectively.[Conclusion]The positive expression rates of OPN,MMP-3 and MMP-10 in colon cancer tissues are significantly incresed.Three test factors are available for current diagnosis of colon cancer.
