肝复乐联合肝动脉化疗栓塞术治疗肝细胞癌有效性的meta分析

摘　　要：目的系统评价肝复乐联合经导管肝动脉化疗栓塞术(TACE)治疗肝细胞癌的效果。方法计算机检索PubMed、Medline、EMBase、Cochrane Library、CBM、中国知网、万方及VIP数据库,检索时间均为建库至2019年12月。筛选肝复乐联合TACE治疗肝细胞癌的随机对照试验,采用RevMan5.3软件进行meta分析。结果最终纳入11篇文献。Meta分析结果显示:试验组有效率高于对照组(OR=2.05,95%CI:1.41,2.97,P=0.0001)。试验组丙氨酸转氨酶水平低于对照组(MD=-53.86,95%CI:-73.33,-34.38,P<0.00001)。试验组谷氨酰转肽酶水平低于对照组(MD=-142.46,95%CI:-152.88,-132.04,P<0.00001)。试验组甲胎蛋白水平低于对照组(MD=-46.18,95%CI:-55.70,-36.67,P<0.00001)。试验组半年生存率高于对照组(OR=3.65,95%CI:1.57,8.51,P=0.003)。试验组1年生存率高于对照组(OR=2.09,95%CI:1.47,2.96,P<0.0001)。试验组2年生存率高于对照组(OR=2.12,95%CI:1.21,3.72,P=0.008)。结论与单纯TACE治疗比较,肝复乐联合TACE治疗肝细胞癌的临床效果较好,值得推广采用。Objective To systematically evaluate the effect of Ganfule combined with transcatheter arterial chemoembolization(TACE)on hepatocellular carcinoma.Methods PubMed,Medline,EMBase,Cochrane Library,CBM,CNKI,WanFang and VIP databases were all searched by computer,and the retrieval time was set up until December 2019.Randomized controlled trial of Ganfule combined with TACE in the treatment of hepatocellular carcinoma were screened.Meta-analysis was performed using RevMan5.3 software.Results A total of 11 studies were included in the analysis.Meta analysis showed that the effective rate of the experimental group was higher than that of the control group(OR=2.05,95%CI:1.41,2.97,P=0.0001).The alanine aminotransferase level in the experimental group was lower than that in the control group(MD=-53.86,95%CI:-73.33,-34.38,P<0.00001).The level of gamma-glutamyltransferase in the experimental group was lower than that in the control group(MD=-142.46,95%CI:-152.88,-132.04,P<0.00001).The alpha fetal protein level of experimental group was lower than the control group(MD=-46.18,95%CI:-55.70,-36.67,P<0.00001).The half-year survival rate in the experimental group was higher than that in the control group(OR=3.65,95%CI:1.57,8.51,P=0.003).The one-year survival rate in the experimental group was higher than that in the control group(OR=2.09,95%CI:1.47,2.96,P<0.0001).The two-year survival rate in the experimental group was higher than that in the control group(OR=2.12,95%CI:1.21,3.72,P=0.008).Conclusion Compared with TACE alone,Ganfule combined with TACE in the treatment of hepatocellular carcinoma has a better clinical effect,which is worthy of promotion and application.
