摘 要:[目的]分析双歧杆菌三联活菌散对胆汁淤积性肝病(CLD)患儿肠道双歧杆菌、乳酸杆菌和大肠杆菌数量变化及肝功能的影响。[方法]选取148例CLD患儿,随机分为对照组(常规治疗)和联合组(常规治疗联合双歧杆菌三联活菌散),每组各74例。检测2组患儿治疗前后肝功能指标、肠道菌群数量及相关炎症细胞因子水平的变化情况。[结果]2组治疗前后总胆固醇(TC)水平比较差异无统计学意义(P>0.05),而治疗后肝功能指标的水平均低于治疗前(P<0.05)。相比治疗前,联合组治疗后肠道乳酸杆菌和双歧杆菌均明显增多,大肠杆菌数量明显减少(P<0.05)。联合组治疗前后转化生长因子-β1水平并无明显变化(P>0.05);相比治疗前,联合组治疗后IL-6和肿瘤坏死因子-α(TNF-α)水平均明显降低(P<0.05)。相比对照组,联合组治疗后2周血清IL-6和TNF-α水平均明显降低(P<0.05)。联合组治疗后2周临床总有效率与对照组比较差异有统计学意义,联合组优于对照组(P<0.05)。[结论]小儿CLD的发病和病情的进展可能与肠道菌群数量变化有关,并且肠道菌群数量的改变可能会诱发炎症细胞因子水平的变化。采用双歧杆菌三联活菌散治疗不仅可促进CLD患儿肠道菌群的恢复,而且可阻滞炎症细胞因子的释放,抑制炎症反应,值得临床应用。[Objective]To analyze the effect of Bifidobacterium triple viable powder on the quantity change of intestinal bifidobacterium,lactobacillus and escherichia coli and liver function in children with cholestatic liver disease(CLD).[Methods]One hundred and forty-eight children with CLD were randomly divided into control group(routine treatment)and combined group(routine treatment combined with bifidobacterium triple viable powder),74 cases in each group.The changes of liver function index,intestinal microflora and related inflammatory cytokines were detected before and after treatment.[Results]There was no significant difference in total cholesterol(TC)between the two groups before and after treatment(P>0.05),but the level of liver function index after treatment was lower than that before treatment(P<0.05).Compared with before treatment,the number of lactobacilli and bifidobacteria in intestines and the number of escherichia coli in the combined group decreased significantly(P<0.05).The level of transforming growth factor-β1 in the combined group did not change significantly before and after treatment(P>0.05);compared with that before treatment,the levels of IL-6 and tumor necrosis factor-α(TNF-α)in the combined group decreased significantly(P<0.05).Compared with the control group,the serum levels of IL-6 and TNF-αin the combined group were significantly lower 2 weeks after treatment(P<0.05).After 2 weeks of treatment,the clinical total effective rate of the combination group was significantly different from that of the control group,and the combination group was better than the control group(P<0.05).[Conclusion]The development of CLD in children may be related to the change of intestinal flora,and the change of intestinal flora may induce the change of inflammatory cytokines.The treatment of bifidobacterium triple viable powder can not only promote the recovery of intestinal flora of CLD children,but also block the release of inflammatory cytokines,which can inhibit the inflammatory response,so it is worth clini
