摘 要:目的探讨血管内皮生长因子(VEGF)及其含激酶插入区受体(KDR)和RBM5基因在前列腺癌(PCa)中表达的相关性及临床意义。方法通过免疫组化SABC法检测PCa患者组织标本中VEGF,KDR和RBM5的表达情况,初步分析这三种标志物与肿瘤临床分期和肿瘤细胞分化的关系及VEGF/KDR在瘤细胞中的共表达情况,并就其共表达和RBM5的相关性以及临床意义进行对比。结果PCa患者中VEGF,KDR和RBM5的阳性表达率分别为69.4%,56.1%和48.0%。同时提示非胞膜浸润和细胞分化好的PCa患者组织标本中VEGF,KDR和RBM5表达水平高于前列腺胞膜浸润和细胞分化差的,两者相比,差异有统计学意义(P<0.05),VEGF/KDR间表达存在显著相关性(P<0.01),其中共表达率为45.9%(90/196),RBM5与VEGF和KDR阳性表达呈正相关(RBM5vs VEGF,P<0.01;RBM5vs KDR,P<0.01)。随访结果表明,这三种标志物同时高表达者,术后PSA反弹早,肿瘤原位复发和远处骨转移的风险高。结论VEGF/KDR共表达说明在PCa细胞中存在VEGF自分泌现象,RBM5与VEGF/KDR表达的相关性提示,RBM5可能参与了肿瘤的新生血管生成。重视三者的相互影响,有助于提高PCa患者分子靶向用药的疗效。另外,对这三种基因异常高表达的患者,提示术后复发风险高和预后不良,应强化治疗并加强随访。Objective To investigate the correlation and clinical significance of vascular endothelial growth factor(VEGF),its kinase insert domain containing receptor(KDR)and RBM5gene expression in prostate cancer(PCa).Methods The expression of VEGF,KDR and RBM5in tissue samples of PCa patients was detected by immunohistochemical SABC method.The relationship between these three markers and tumor clinical stage and tumor cell differentiation and the co-expression of VEGF/KDR in tumor cells were preliminarily discussed and analyzed,and the correlation between their co-expression and RBM5and the clinical significance were compared.Results The positive expression rates of VEGF,KDR and RBM5in PCa patients were 69.4%,56.1%and 48.0%,respectively.Also reminded the capsular invasion and cell differentiation in patients with PCa tissue samples of VEGF,KDR and RBM5expression level above the cell membrane of prostate invasion and poor differentiation,both,compared to difference was statistically significant(P<0.05),between VEGF/KDR expression exist significant correlation(P<0.01),of which the total expression rate was 45.9%(90/196),positive correlation between VEGF and KDR positive expression and RBM5(RBM5vs VEGF,P<0.01;RBM5vs KDR,P<0.01).Follow-up results showed that those with high expression of these three markers at the same time had early postoperative PSA rebound,high risk of tumor recurrence in situ and distant bone metastasis.Conclusion The co-expression of VEGF/KDR indicates the existence of VEGF autocrine in PCa cells,and the correlation between RBM5and VEGF/KDR expression suggests that RBM5may be involved in tumor angiogenesis.It is helpful to improve the efficacy of molecular targeted drug for PCa patients to attach importance to the interaction of the three.In addition,patients with abnormally high expression of these three genes are suggested to have a high risk of postoperative recurrence and poor prognosis,and should be strengthened in treatment and follow-up.
