ProGRP、SCC-Ag、CYFRA21-1对小细胞肺癌的诊断价值分析

摘　　要：目的分析胃泌素释放肽前体(ProGRP)、鳞癌相关抗原(SCC-Ag)、细胞角蛋白-19片段(CYFRA21-1)对小细胞肺癌的临床诊断价值。方法选取小细胞肺癌(SCLC)初诊患者100例作为SCLC组,同期肺部良性病变患者100例作为良性病变组,同期进行健康体检者100例作为健康对照组。分别检测三组受试者的ProGRP、SCC-Ag、CYFRA21-1水平。结果SCLC组患者的ProGRP、SCC-Ag和CYFRA21-1水平均显著高于良性病变组和健康对照组,组间比较具有统计学差异(P<0.05),良性病变组与健康对照组的上述指标比较无统计学差异(P>0.05);随着临床分期的增加,SCLC患者的ProGRP、SCC-Ag和CYFRA21-1水平显著升高,四组间比较具有统计学差异(P<0.05);ProGRP诊断SCLC的灵敏度和特异度均显著高于SCC-Ag和CYFRA21-1(P<0.05),三者联合检测的特异度最高为100%。结论ProGRP、SCC-Ag、CYFRA21-1可作为诊断小细胞肺癌的标志物,但ProGRP较SCC-Ag和CYFRA21-1诊断SCLC的灵敏度和特异性更高,三者联合检测的效果更好。Objective To analyze the clinical diagnostic value of ProGRP、SCC-Ag and CYFRA21-1 in small cell lung cancer.Methods 100 newly diagnosed patients with small cell lung cancer(SCLC)were selected as the SCLC group,100 patients with benign lung lesions during the same period as the benign lesions group,and 100 patients with healthy physical examination as the healthy control group.The levels of ProGRP,SCC-Ag and CYFRA21-1 were detected in the 3 groups.Results The ProGRP、SCC-Ag and CYFRA21-1 levels of patients in SCLC group were significantly higher than those in the benign lesion group and the healthy control group,with statistically significant differences among the 3 groups(P<0.05),and no statistically significant differences in the above-mentioned indicators between the benign lesion group and the healthy control group(P>0.05).With the increase of clinical stages,the ProGRP、SCC-Ag and CYFRA21-1 levels of SCLC patients increased significantly,and there was statistical difference among the 4 groups(P<0.05).The sensitivity and specificity of ProGRP in diagnosing SCLC were significantly higher than those of SCC-Ag and CYFRA21-1(P<0.05),and the maximum specificity of the combined detection of the three was 100%.Conclusion ProGRP、SCC-Ag and CYFRA21-1 can be used as markers in the diagnosis of small-cell lung cancer,but ProGRP has a higher sensitivity and specificity in the diagnosis of SCLC than scc-ag and cyfra21-1.The combined detection of the three is better.
