急诊血钾值预测高血压脑出血患者并发应激性溃疡出血的临床意义

摘　　要：目的分析高血压脑出血患者入院时即刻血钾水平指导临床上预测高血压脑出血后应激性溃疡是否易并发出血的临床意义。方法收集德阳市人民医院2017年1月至2018年12月256例高血压性脑出血患者的临床资料,其中发生应激性溃疡出血33例,在从未发生溃疡出血患者中随机选取35例。比较两组血钾水平,分析其与溃疡出血的关系。结果溃疡组与非溃疡组血钾水平比较,差异有统计学意义(t=-7.54,P<0.05),其中溃疡出血组血钾水平为(3.07±0.33)mmol/L,明显低于溃疡未出血组的(3.79±0.44)mmol/L,二分类logistics回归分析结果显示,血钾水平是溃疡并出血的危险因素(P<0.05,OR=0.006),即入院急诊血钾越低溃疡出血的风险越大。结论高血压脑出血的患者入院时血钾水平低,可能是患者易发生应激性溃疡出血的一个警示因素,可为临床医生关于高血压脑出血后预防应激性溃疡出血的治疗提供一定的借鉴作用。Objective To analyze the level of serum potassium in patients with hypertensive intracerebral hemorrhage at admission,so as to guide the clinical prediction of stress ulcer after hypertensive intracerebral hemorrhage.Methods A total of 256 patients with hypertensive intracerebral hemorrhage from January 2017 to December 2018 in the People’s Hospital of Deyang were enrolled.Among them,33 cases had stress ulcer bleeding,and 35 cases had never occurred ulcer bleeding.The serum levels of potassium were compared between the two groups,and the relationship between serum potassiam levels and ulcer bleeding was analyzed.Results There was significant difference in serum potassium levels between the ulcer group and the non-ulcer group(t=-7.54,P<0.05).The serum potassium level in ulcer bleeding group was(3.07±0.33)mmol/L,which was significantly lower than that in ulcer non bleeding group[(3.79±0.44)mmol/L].The results of binary logistic regression analysis showed that serum potassium level was a risk factor for ulcer bleeding(P<0.05,OR=0.006),that is,the lower the blood potassium level,the greater the risk of ulcer bleeding.Conclusions The lower the serum potassium level in patients with hypertensive intracerebral hemorrhage,the more prone to stress ulcers.It is a risk factor for patients to be susceptible to stress ulcers.It should be vigilant and prevent early.
