中重度睡眠呼吸暂停综合征患者冠状动脉造影特点分析

摘　　要：目的:探析对中重度睡眠呼吸暂停综合征合并冠心病患者应用冠状动脉造影检查的临床诊断指导价值。方法:选取2017—2019年新疆军区总医院收治的合并有中重度睡眠暂停综合征的冠心病患者30例作为观察组,另选同期收治单纯冠心病患者30例作为对照组,所有患者均接受冠状动脉造影检查并使用酶联免疫法对其血浆内皮素-1(ET-1)、C反应蛋白(CRP)浓度进行检测,对其临床数据以回顾性方式分析并归纳其临床诊断价值。结果:相对于对照组,观察组患者在冠状动脉血管病变数量、闭塞性的病变数量、血管严重迂曲、血管钙化以及有桥侧枝情况均相对更高(P<0.05);存在闭塞情况的血管长度观察组患者相对更高,在TIMI血流状况的差异有统计学意义(P<0.05);观察组患者接受血浆ET-1以及CRP检测后数据均显著高于对照组,差异有统计学意义(P<0.05)。结论:中重度睡眠呼吸暂停综合征合并冠心病患者在冠状动脉病变方面表现更为明显,这与其内皮功能受损、炎性反应严重存在联系。Objective:To explore the clinical diagnostic value of coronary angiography in patients with moderate and severe sleep apnea syndrome and coronary heart disease.Methods:From 2017 to 2019,30 patients with coronary heart disease complicated with moderate or severe sleep apnea syndrome were selected and set as the observation group,and another 30 patients with simple coronary heart disease were selected and set as the control group.All patients were examined by coronary angiography and their plasma concentrations of endothelin-1(ET-1)and C-reactive protein(CRP)were measured by enzyme-linked immunosorbent assay The clinical data were analyzed retrospectively and the clinical diagnosis value was concluded.Results:Compared with the control group,the number of coronary artery lesions,occlusive lesions,serious vascular tortuosity,vascular calcification and bridged collateral were higher in the observation group(P The data of ET-1 and CRP were significantly higher than that of the control group(P<0.05).Conclusion:Patients with moderate or severe sleep apnea syndrome and coronary heart disease have more obvious coronary artery disease,which is related to endothelial dysfunction and inflammatory response.
