泪道内窥镜检查在诊断泪小管炎中的应用

摘　　要：目的通过泪道内窥镜了解疑似泪小管炎患者泪小管内的情况,并同时查看泪道内是否存在其他病变。方法收集2017年8月至2019年5月唐山市眼科医院门诊就诊的有眼分泌物增多症状,经裂隙灯检查泪点或冲洗泪道后高度怀疑泪小管炎的患者109例(109只眼),均在局部麻醉下行泪道内窥镜检查,观察泪小管内情况,并确定泪道内其他部位是否存在病变。结果泪道内窥镜发现,泪小管内存在息肉者有40例(36.70%)、结石人数有23例(21.10%)、结石合并息肉人数有36例(33.03%),检查结果阳性率达90.83%。同时发现,确诊为泪小管炎的患者中存在泪小管阻塞的18例(18.18%)、鼻泪管阻塞11例(11.11%)、鼻泪管狭窄36例(36.36%),检查结果阳性率达65.66%。结论泪道内窥镜检查直视下能够观察泪小管内情况并同时发现泪道内的其他病变,对泪小管炎的诊断和手术方案的制定具有重要意义。OBJECTIVE To investigate the condition of lacrimal ductule in patients with suspected canaliculitis by lacrimal duct endoscopy,and to examine whether other lesions in lacrimal duct.METHODS One hundred and nine patients(109 eyes)with eye secretion overabundance who were admitted to our ophthalmic clinic were selected from August 2017 to May 2019.After examining puncta lacrimalia by slit lamp and washing lacrimal duct,canaliculitis was highly suspected.The lacrimal duct endoscopic examination was performed under local anesthesia to observe the condition of lacrimal duct and to find lesions in other parts of the lacrimal duct.RESULTS Forty cases(36.70%)with lacrimal duct polyps were found by lacrimal duct endoscope,23 cases(21.10%)with lithiasis,and 36 cases(33.03%)with lithiasis and polyps.The positive rate of the examination results was 90.83%.Meanwhile,there were 18 cases(18.18%)with lacrimal duct obstruction,11 cases(11.11%)with nasolacrimal duct obstruction,36 cases(36.36%)with nasolacrimal duct stenosis in patients diagnosed as canaliculitis,and the positive rate of examination results reached 65.66%.CONCLUSIONS Lacrimal duct endoscopy could be used to observe the condition of lacrimal ductule directly and find other lesions in lacrimal duct at the same time,which is of great significance for the diagnosis and surgical planning formulation of canaliculitis.
