摘　　要：目的探讨3.0T MRI在三阴性乳腺癌诊断中的应用价值。方法抽取2018年1月至2020年1月河南科技大学第一附属医院收治的乳腺癌患者135例,均行3.0T MRI检查,以病理结果为金标准统计3.0T MRI诊断效能,分析三阴性乳腺癌的影像特征。结果135例乳腺癌患者经病理确诊为三阴性乳腺癌98例,3.0T MRI检出88例。3.0T MRI诊断三阴性乳腺癌灵敏度为89.80%(88/98),特异度为91.89%(34/37),阳性预测值为96.70%(88/91),阴性预测值为77.27%(34/44),准确率为90.37%(122/135)。其诊断效能与病理结果一致性参数Kappa=0.771,P<0.01。3.0T MRI影像见三阴性乳腺癌肿块呈圆形、椭圆形,边缘清晰、可见毛刺,肿块后方回声增强或无变化。结论3.0T MRI对三阴性乳腺癌诊断准确性较高,且与病理检查一致性较好,有一定临床价值。Objective To investigate the application value of 3.0T MRI in the diagnosis of triple negative breast cancer.Methods A total of 135 breast cancer patients admitted to the First Affiliated Hospital of Henan University of Science and Technology from January 2018 to January 2020 were selected,and all of them underwent 3.0T MRI.The pathological results were used as the gold standard to calculate the diagnostic efficiency of 3.0T MRI,and the imaging characteristics of triple negative breast cancer were analyzed.Results Among the 135 breast cancer patients,98 cases were pathologically diagnosed with triple negative breast cancer,and 88 cases were detected by 3.0TMRI.The sensitivity of 3.0T MRI in diagnosing triple-negative breast cancer was 89.80%(88/98),the sensitivity of 3.0T MRI in diagnosing triple negative breast cancer was 89.80%(88/98),the specificity was 91.89%(34/37),the positive predictive value was 96.70%(88/91),and the negative predictive value was 77.27%(34/44),the accuracy rate was 90.37%(122/135).The consistency parameter between the diagnostic efficiency and the pathological results was Kappa=0.771,P<0.01.3.0T MRI showed that the triple-negative breast cancer mass was round or oval,with clear edges or visible burrs,and enhanced or unchanged echoes behind the mass.Conclusions 3.0T MRI has high accuracy in the diagnosis of triple negative breast cancer,and good consistency with pathological examination,and has certain clinical value.
