乳腺癌放疗中两种体位固定技术的应用比较

摘　　要：目的:比较乳腺癌放疗中使用乳腺托架和发泡胶两种固定技术的摆位精度。方法:随机选取2019年1月至6月在中山大学肿瘤防治中心接受放射治疗的40例乳腺癌患者平均分成两组:A组患者均用CIVCO公司生产的乳腺托架固定体位;B组患者均用发泡胶固定体位。所有患者均在治疗前于医科达ELEKTA Synergy直线加速器的电子射野影像系统(EPID)下拍摄5次正、侧位射野验证片(每周一次),并与CT模拟定位图数字化重建影像(DRR)进行对比,得出摆位位移数据,比较各组摆位误差的不同。结果:两组患者各个方向的摆位误差分别为:左右(X轴):(2.82±1.61)mm和(2.30±1.26)mm(P<0.05):头脚(Y轴):(2.50±1.53)mm和(1.61±1.13)mm(P<0.05);腹背(Z轴):(3.05±1.54)mm和(1.75±1.17)mm(P<0.05)。结论:在每周一次的EPID验证条件下,用发泡胶固定的B组患者在左右(X轴)、头脚(Y轴)及腹背(Z轴)三个方向上的误差均小于使用乳腺托架固定的A组患者。Objective:Comparison of the positioning accuracy of breast cancer brace and styrofoam fixation techniques in breast cancer radiotherapy.Methods:Forty breast cancer patients who underwent radiation therapy at Sun Yat-sen University Cancer Center from January to June 2019 were randomly divided into two groups: group A patients were fixed with breast brackets produced by CIVCO;group B patients Position yourself with styrofoam. All patients underwent 5 positive and lateral field verification films(once a week) under Elekta Synergy’s Electronic Field Imaging System(EPID) before treatment, and digitally reconstructed images with CT simulation positioning map DRR) to obtain the position displacement data and compare the differences in the position errors of each group.Results:The positioning errors in each direction of the two groups of patients were: left and right(X axis):(2.82±1.61) mm and(2.30±1.26) mm(P < 0.05): head and foot(Y axis):(2.50±1.53) Mm and(1.61±1.13) mm(P<0.05);abdomen(Z axis):(3.05±1.54) mm and(1.75±1.17) mm(P<0.05).Conclusion:Under the condition of weekly EPID verification, the errors in the three directions of left and right(X axis), head and foot(Y axis), and abdomen(Z axis) of group B patients fixed with styrofoam are smaller than those of the breast. Group A patients with brackets.
