肾上腺偶发瘤与颈动脉内膜中层厚度和代谢综合征的相关性分析

摘　　要：目的探讨肾上腺偶发瘤(AI)与颈动脉内膜中层厚度(IMT)和代谢综合征的相关性。方法选取2016年6月至2018年6月在本院诊断为AI的患者90例(观察组),其中28例AI患者被检出患有亚临床库欣综合征(sCS)(sCS组),其余62例患者被诊断为非功能性肾上腺肿瘤(NFAT)(NFAT组)。另选同时期在本院体检中心行健康体检者50例作为对照组。对所有患者使用抗胰岛素性稳态模式评估法(HOMA-IR)和过夜小剂量地塞米松抑制试验(1 mg DST)进行胰岛素测定和激素定量,对患者的超声、心动图相关指标及IMT进行统计分析。结果在血生化及代谢指标方面,观察组患者的血浆葡萄糖、胰岛素和三酰甘油值均明显高于对照组(P<0.05)。超声心动图及颈动脉彩超结果发现,观察组患者的心率、舒张末期直径、收缩末期直径和IMT均明显高于对照组(P<0.05)。结论尽管AI大多数是无功能性的,但仍会增加患者胰岛素抵抗、心血管疾病风险、IMT和舒张功能参数。Objective To explore the association between adrenal incidental tumors and carotid intima-media tliickness(IMT)and metabolic syndrome1.Methods From 2016 to 2018,a total of 90 patients who were diagnosed with adrenal incidental tumors in our hospital were selected as the experimental group.All patients were Ireated wilh insulin resistance index(HOMA-IR)and overnight lowdose dexametliasone suppression test(1 mg-DST).Insulin determination and hormone quantification were performed,and the echocardiographic relateud indicators and carotid intima-media thickness were counted.Fifty healthy people who were in the physical examination center of our hospital at the same time were selected as the control group.The two groups of subjects were strictly matched in tenns of age,gender,and body mass index(BMI).Results Of the 90 pcitients with adrenal imitlentcil tumors,28 were found to have subclinical Cushings syndrome(sCS)and the remaining 62 were diagnosed as non-functional adrenal adenomas.In terms of blood biochemistn and metabolic parameters,plasma glucose(P=0.010),insulin(P=0.005)and triglyceride(P=0.012)values were significantly higher in idl patients than in the control group.Kchocardiography and carotid ultrasound showed that heart rate(P=0.005),end-diastolic diameter(P=0.040),end-systolic diameter(P=0.021),and carotid intima-media thickness(P=0.003)were all in all patients.There was significantly higher than the control group.Conclusions Although most of the adrenal incidents are non-functional,they still lead to increased insulin resistance,increased risk of cardiovascular disease,and increased carotid intimamedia thickness and diastolic func tion parameters.
