眶隔筋膜瓣与额肌瓣吻合悬吊矫正治疗重度上睑下垂的矫正效果及术后并发症观察

摘　　要：目的探究采用眶隔筋膜瓣与额肌瓣吻合悬吊矫正治疗重度上睑下垂的矫正效果及术后并发症发生情况。方法 80例重度上睑下垂患者,依据治疗方法的不同将患者分为常规组与研究组,各40例。常规组采用额肌瓣悬吊矫正术治疗,研究组采用眶隔筋膜瓣与额肌瓣吻合悬吊矫正术治疗。比较两组患者的矫正效果、术后不同时间的上睑缩回量和术后并发症发生情况。结果研究组矫正总有效率97.50%明显高于常规组的80.00%,差异具有统计学意义(P<0.05)。术后3、6个月,研究组的上睑缩回量分别为(0.25±0.09)、(0.28±0.08)mm,明显优于常规组的(0.33±0.11)、(0.42±0.10)mm,差异具有统计学意义(P<0.05)。研究组术后并发症发生率5.00%明显低于常规组的22.50%,差异具有统计学意义(P<0.05)。结论对重度上睑下垂患者,要提高矫正效果,减少术后的并发症,采用眶隔筋膜瓣与额肌瓣温和悬吊矫正治疗的效果好,值得推广。Objective To investigate the corrective effect and postoperative complications of orbital septal fascia flap and frontalis muscle flap on suspension correction for severe blepharoptosis. Methods A total of 80 patients with severe blepharoptosis were divided into conventional group and research group according to different treatment methods, with 40 cases in each group. The conventional group was treated with frontal muscle flap for suspension correction, and the research group was treated with orbital septal fascia flap and frontalis muscle flap for suspension correction. The correction effect, amount of upper eyelid retraction at different time after operation and occurrence of complications after operation were compared between the two groups. Results The total effective rate of correction 97.50% of the research group was obviously higher than 80.00% of the conventional group, and the difference was statistically significant(P<0.05). 3 and 6 months after operation, amount of upper eyelid retraction of the research group were(0.25±0.09) and(0.28±0.08) mm, which were better than(0.33±0.11) and(0.42±0.10) mm of the conventional group, and the difference was statistically significant(P<0.05). The incidence of complications after operation 5.00% of the research group was obviously lower than 22.50% of the conventional group, and the difference was statistically significant(P<0.05). Conclusion Orbital septal fascia flap and frontalis muscle flap for suspension correction show good effect for patients with severe blepharoptosis, which can improve the correction effect, reduce postoperative complications, and is worthy of promotion.
