初步探讨T1 mapping序列与肝细胞癌临床病理特征的关系

摘　　要：目的初步探讨平扫T1 mapping成像(自由呼吸Look-locker序列)所得T1值与肝细胞癌(HCC)病理特征的关系.方法对129例经手术病理证实为HCC的患者行Look-locker序列扫描,结合常规平扫+多期增强图像,测量肿瘤实质部分的T1值.分析肿瘤T1值与正常肝脏组织的差异以及T1值在不同分化程度的HCC、有无门静脉癌栓、预测癌肿大小等临床病理特征中的区别,并研究T1值与HCC临床病理特征的关系.结果T1值在HCC与肝脏正常组织之间的差异有统计学意义(P<0.001).T1值在HCC分化不同组之间、是否伴有门静脉癌栓组之间、癌肿>3 cm和≤3 cm组之间有差异(P值分别为0.02、0.025、0.000).多重比较显示肿瘤分化程度高分化组与低分化组之间T1值差异有统计学意义(P=0.023),而中分化组与高分化组、低分化组T1值之间差异均无统计学意义(P=0.184、0.222).结论T1 mapping序列能够预测HCC术前病理分级,T1值对于初步预判是否伴有门静脉癌栓、提前评估癌肿的大小具有一定的作用.Objective To investigate the relationship between T1values of non-enhanced T1mapping(free-breathing Look-locker sequence)and the clinicopathologic characteristics of hepatocellular carcinoma(HCC).Methods T1mapping was performed in 129 patients with HCC confirmed by pathology.T1values of the solid portion of the tumor were measured,which were confirmed by reviewing all the pre-and post-contrast MR images.The differences in T1value between tumor and normal liver tissue,between the patients with or without portal tumor thrombus,among different differentiation degrees and leison sizes were analyzed.Results The significant differences in T1value were found between HCC and normal liver tissue(P<0.001),among different differentiation degrees(P=0.025),between patients with and without portal tumor thrombus(P=0.025),and between lesions〉3 cm and≤3 cm(P=0.000).Multiple comparisons showed a statistically significant difference in Ti values between the well-and the poorly-differentiated HCC(P=0.023),however,no statistical differences between other differentiated leisons were found (P=0.814,0.222).Conclusion T1value has a preoperative predictional ability for pathological grading of HCC,and may help for the assessment of portal tumor thrombus and the lesion size.
