摘 要:目的利用光学相干断层扫描血管成像(optical coherence tomography angiography,OCTA)观察正常眼压性青光眼(normal tension glaucoma,NTG)视盘区血管密度变化,并探讨其与NTG临床分期的相关性。方法选取2019年1月至2020年5月在我院眼科确诊的NTG患者30例为NTG组,同期选择正常志愿者30人为对照组。采用OCTA对所有受检者视盘区进行扫描,测量视盘周围血管密度,比较两组差异,并分析NTG视盘区域血管密度与青光眼临床分期的相关性。结果与对照组相比,NTG组视盘区域大血管、毛细血管和全区域血管密度均明显下降,差异均有统计学意义(均为P<0.01),而其毛细血管间隙和无血管区密度均显著高于对照组(均为P<0.01);NTG组视盘区域大血管、毛细血管和全区域血管密度与临床分期均呈负相关(r大血管=-0.357、r毛细血管=-0.965、r全区域血管=-0.643,均为P<0.001),而毛细血管间隙和无血管区密度与临床分期呈正相关(r毛细血管间隙=0.538、r无血管区=0.706,均为P<0.001)。结论利用OCTA可以方便无创监测NTG眼底结构和功能变化,且这种改变与青光眼临床分期相关,OCTA检测在NTG的诊断和随访中均具有重要的意义。Objective To study the change of optic disc blood flow in normal-tension glaucoma(NTG)by optical coherence tomography angiography(OCTA),and discuss the correlation between optic disc blood flow and NTG clinical stages.Methods Together 30 patients of NTG confirmed in the Department of Ophthalmology in the First Affiliated Hospital of Nanchang University were selected as the experimental group and 30 normal people as the control group.All subjects underwent the OCTA scan test to obtain the blood vessel density around the optic disc.The blood vessel density change region between NTG group and the control group was compared,and the correlation between the changes of optic disc blood flow density and NTG clinical stages was tested.Results Compared with the control group,the density of large vessels,capillaries and blood vessels around the optic disc in the NTG group significantly decreased(all P<0.01),while the density of capillary space and the avascular area were significantly increased(both P<0.01).The density of optic disc large vessels,capillaries,and whole area blood vessel density in the NTG group were negatively correlated with NTG clinical stage(rlarge vessels=0.357,rcapillaries=0.965,rblood vessels=0.643,all P<0.001),while capillary space and avascular density in the NTG group were positively correlated with NTG clinical stages(rcapillary space=0.538,ravascular area=0.706,all P<0.001).Conclusion OCTA can facilitate the non-invasive monitoring of changes in the structure and function of the fundus of NTG,and this change is related to the clinical staging of glaucoma.OCTA detection is of great significance in the diagnosis and follow-up of NTG.
