摘 要:目的系统比较电子胃镜、CT和超声内镜检查在上消化道黏膜下隆起型病变筛查和诊断中的临床价值,以期为临床筛查和诊断提供理论指导。方法本研究回顾性分析了2018年1月—2019年12月收治于我院消化内科中心上消化道黏膜下隆起型病变行ESD手术的患者中随机选取的126例,通过对比电子胃镜和CT检查两者对上消化道黏膜下隆起型病变的临床筛查准确率,并对筛查问题病例进行超声内镜检查诊断,评估超声内镜诊断准确率,以期为临床选择上消化道黏膜下隆起型病变的筛查和诊断手段提供理论参考。结果对于上消化道黏膜下隆起型病变的诊断,电子胃镜的筛查准确率为100%(109/109),CT检查的筛查准确率为50.0%(48/96),电子胃镜的筛查诊断准确率高于CT(P<0.05),将电子胃镜筛查问题病例使用超声内镜诊断,以术后病理为金标准,CT诊断准确率30.2%(29/96),超声内镜诊断准确率高达72.6%(90/124)。结论使用电子胃镜与超声内镜联用对上消化道黏膜下隆起型病变的筛查和诊断具有重要的临床意义。Objective To systematically compare the clinical value of electronic gastroscopy,CT and endoscopic ultrasonography in the screening and diagnosis of upper gastrointestinal submucosal eminence lesions,so as to provide theoretical guidance for clinical screening and diagnosis.Methods This study retrospectively analyzed between January 2018 and December 2019 treated in our hospital digests medical center line type upper gastrointestinal submucosal apophysis lesions ESD surgery in patients randomly selected from 126 cases,by comparing both the electronic gastroscope and CT examination of upper gastrointestinal submucosal apophysis lesions of clinical screening accuracy,and check for screening problem cases endoscopic ultrasonography diagnosis,assessment of endoscopic ultrasonography in the diagnosis of accuracy,in order to clinical choice type upper gastrointestinal submucosal apophysis lesions of screening and diagnostic tools to provide theoretical reference.Results The diagnosis of type swell for upper gastrointestinal submucosal lesions,electronic gastroscope screening accuracy rate was 100%(109/109),screening accuracy of CT examination was 50.0%(48/96),the electronic gastroscope screening diagnosis accuracy is significantly higher than CT(P<0.05),the electronic gastroscope screening problem using endoscopic ultrasonography in the diagnosis of cases,with the postoperative pathology as the gold standard,the CT diagnosis accuracy rate 30.2%(29/96),endoscopic ultrasonography diagnosis accuracy rate is as high as 72.6%(90/124).Conclusion The combination of electronic gastroscope and endoscopic ultrasonography has important clinical significance in the screening and diagnosis of upper gastrointestinal submucosal eminence lesions.
