摘 要:目的探讨卵巢癌化疗后口腔黏膜炎的影响因素并构建相关模型。方法选择2018年1月至2019年12月于温州医科大学附属第二医院进行化疗的226例卵巢癌患者作为研究对象,以患者化疗后是否发生口腔黏膜炎作为分组依据分为发生组86例和未发生组140例,对两组患者的临床资料进行单因素和多因素Logistic回归分析,建立预测模型并评价预测效果。结果年龄、吸烟史、口腔清洁度、含5-氟尿嘧啶(5-FU)化疗方案是卵巢癌化疗后发生口腔黏膜炎的影响因素,其OR值及95%CI分别为2.173(1.134~3.675)、2.824(1.285~6.205)、1.702(1.027~3.231)、2.207(1.229~3.962),均P<0.05;卵巢癌化疗后发生口腔粘膜炎的概率预测模型为Logit(P)=-27.846+0.776×年龄+1.038×吸烟史+0.532×口腔清洁度+0.791×化疗方案,模型的灵敏度为81.13%,特异度为92.31%,受试者工作特征(ROC)曲线下面积为0.966,预测正确率为86.73%。结论年龄、吸烟史、口腔清洁度、含5-FU化疗方案是卵巢癌患者化疗后发生口腔黏膜炎的影响因素,应加强对患者的口腔健康教育,劝诫患者禁烟,并建议患者慎重选择含5-FU的化疗方案治疗。Objective To explore the influencing factors of oral mucositis after chemotherapy for ovarian cancer and to construct related models.Methods 226 patients with ovarian cancer receiving chemotherapy from January 2018 to December 2019 in the Second Affiliated Hospital of Wenzhou Medical University were selected as the research objects.According to the occurrence of oral mucositis after chemotherapy,the patients were divided into 86 cases in the occurrence group and 140 cases in the non-occurrence group.Univariate and multivariate Logistic regression analysis were performed on the clinical data of the two groups to establish a prediction model and evaluate its prediction effect.Results Age,smoking history,oral cavity cleanness and 5-fluorouracil(5-FU)chemotherapy regimens were the influence factors of oral cavity mucous membrane inflammation after the ovarian cancer chemotherapy,with OR values and their 95%CI were 2.173(1.134-3.675),2.824(1.285-6.205),1.702(1.027-3.231)and 2.207(1.229-3.962),all P<0.05.The probabilistic prediction model of oral mucositis after chemotherapy for ovarian cancer was Logit(P)=-27.846+0.776×age+1.038×smoking history+0.532×oral cleanliness+0.791×chemotherapy regimen.The model’s sensitivity and specificity were 81.13%and 92.31%,respectively.The area under ROC curve was 0.966,and the prediction accuracy was 86.73%.Conclusion Age,smoking history,oral cleanliness,and 5-FU-containing chemotherapy regimen are the influencing factors of oral mucositis after chemotherapy in patients with ovarian cancer.The oral health education of patients should be strengthened,patients should be advised to ban smoking,and choose chemotherapy with 5-FU carefully.
