摘 要:目的探讨基于Sina的增强现实技术在椎管肿瘤手术定位中的应用价值。方法回顾性分析2019年1—12月于北京大学第三医院神经外科行手术治疗的20例椎管肿瘤患者的临床资料。术前随机分为Sina定位组(10例)和传统体表解剖定位组(10例),两组术前均定位肿瘤的体表投影。Sina定位组采用基于Sina的增强现实技术,传统体表解剖定位组仅参考体表解剖标记。应用术中C型臂分别验证两组的定位结果,并与MRI显示的肿瘤实际位置进行比较。比较两组患者的定位误差、总定位用时和C型臂投照次数。结果Sina定位组的总定位用时[(12.5±3.0)min]较传统体表解剖定位组[(9.3±1.7)min]长,两组的差异有统计学意义(t=2.913,P=0.009)。Sina定位组的定位误差[(2.98±0.92)cm]明显小于传统体表解剖定位组[(4.39±1.87)cm](t=-2.131,P=0.047),且Sina定位组的C型臂投照次数[(1.3±0.5)次]亦少于传统体表解剖定位组[(2.2±0.8)次](t=-3.077,P=0.006)。结论采用基于Sina的增强现实技术进行椎管肿瘤定位安全、精准。然而,Sina的定位准确性仍需更多的临床实践进一步明确。Objective To explore the application value of augmented reality based on Sina in the localization of spinal canal tumors.Methods The clinical data of 20 patients with spinal canal tumors who underwent surgical treatment at Neurosurgery Department of Peking University Third Hospital from January 2019 to December 2019 were retrospectively analyzed.S-group(based on Sina,10 cases)and A-group(based on anatomical markers,10 cases)were randomly divided.Before surgery,the skin projection of tumor was located in both groups.Augmented reality method based on Sina was applied in S-group,while the anatomical markers on the body surface were the only references in A-group.The localization results of the two groups were verified using intraoperative C-arm and compared with the actual location of the tumor shown on MRI.The deviation,total localization time and times of C-arm used were documented and compared between two groups.Results The total localization time in S-group(12.5±3.0 min)was longer than that in A-group(9.3±1.7 min)(t=2.913,P=0.009).The localization deviation in S-group(2.98±0.92 cm)was significantly less than that in A-group(4.39±1.87 cm)(t=-2.131,P=0.047),and times of C-arm used(1.3±0.5)in S-group was less than that in A-group(2.2±0.8)(t=-3.077,P=0.006).Conclusions It is safe and precise to localize spinal canal tumor using augmented reality based on Sina.However,the accuracy of Sina needs further verification by more clinical practice.
