摘　　要：目的:探讨PDCA循环在持续改进医保自费项目知情同意书签署管理中的实践效果。方法:抽取改进前病历257份,分析《自费项目知情同意书》签署存在的问题,运用PDCA循环持续改进。抽取改进后病历1901份,比较改进前后《自费项目知情同意书》签署情况、患者医疗负担情况及患者就医满意度。结果:改进后《自费项目知情同意书》签署合格率明显提高,患者自费费用占总费用比例下降、满意度提高。结论:PDCA循环能持续改进《自费项目知情同意书》签署合格率,有利于降低患者自费负担,维护患者的合法权益,提升医院医保管理质量。Objective:To explore the application and eff ect of PDCA cycle in continuously improving the management of informed consent form for medical insurance self-funded items.Methods:Before the improvement,we extracted 257 medical records to analyze the problems of signing the informed consent form and advancing quality amelioration continuously by PDCA cycle.After the implementation of the improvement,a total of 1901 medical records were collected to compare the patient's medical burden,and the patient's satisfaction with medical treatment before and after implementation.Results:The qualifi ed rate of signing informed consent form and the patient's satisfaction was gradually increased.In addition,patient satisfaction has improved.Conclusion:The PDCA cycle can continuously improve the passing rate of signing the informed consent form for self-funded items,which is conducive to reducing the proportion of patients'out-of-pocket expenses and improving patient's satisfaction and the quality of hospital medical insurance management.
