MSCT血管造影三维重建在颅内动脉瘤诊断中的应用价值探析

摘　　要：目的:评价多层螺旋CT(MSCT)血管造影(CTA)三维重建在颅内动脉瘤(IA)诊断中的应用价值。方法:回顾性分析2017年8月—2019年8月于我院就诊的76例IA患者的临床资料。所有患者均行多层螺旋CT血管造影(MSCTA)及数字血管剪影造影(DSA)检查,以手术结果为金标准。术前对两组检查图像资料进行三维重建,并模拟手术入路。比较MSCTA及DSA检查对IA的检出情况,并比较两种检查方法的图像质量结果及检查时间、检查费用。结果:两种检查方法对IA检出率比较差异无统计学意义(P>0.05)。在IA瘤体大小分布情况上,两种检查方法对各种直径大小动脉瘤检出上差异无统计学意义(P>0.05);MSCTA在显示IA形态方面差异无统计学意义(P>0.05);MSCTA在三维关系、载瘤动脉及瘤体内是否存在血栓、钙化的显示方面优于DSA(P均<0.05);MSCTA检查时间及检查费用明显低于DSA(P均<0.05)。结论:MSCTA及三维重建诊断价值较高,能清晰显示颅内肿瘤位置、大小等情况,且检查便捷,具有费用低、无创等特点。Objective:To evaluate the application value of three-dimensional reconstruction of multi-slice spiral CT(MSCT)angiography(CTA)in the diagnosis of intracranial aneurysms(IA).Methods:The clinical data of 76 patients with IA who were treated in our hospital from August 2017 to August 2019 were retrospectively analyzed.All patients underwent multi-slice spiral CT angiography(MSCTA)and digital subtraction angiography(DSA),with the surgical results as the gold standard.Before surgery,three-dimensional reconstruction was performed on image data of the two examinations,and the surgical approach was simulated.The detection of IA by MSCTA and DSA was compared,and the image quality results,examination time and examination cost of the two examinations were compared.Results:There was no significant difference in the detection rate of IA between the two examinations(P>0.05).In terms of IA tumor size distribution,there was no significant difference in the detection of aneurysms of various diameters between the two examinations(P>0.05).MSCTA showed no statistically significant difference in the morphology of IA(P>0.05).MSCTA was superior to DSA in the displays of three-dimensional relationship,presence or absence of thrombus and calcification in tumor-bearing arteries and tumor bodies(P<0.05).The examination time and examination cost of MSCTA were significantly lower than those of DSA(P<0.05).Conclusion:MSCTA and three-dimensional reconstruction are of high diagnostic value,and can clearly show the location and size of intracranial tumors,and they are convenient examinations with low cost and non-invasion.
