细胞因子与CEA检测在结直肠癌诊断及临床病理特征中的相关性分析

摘　　要：目的探讨血清多种细胞因子在结直肠癌(CRC)中的表达,评估其单独与联合癌胚抗原(CEA)检测对结直肠癌的诊断价值。方法选取确诊的101例结直肠癌患者作为病例组(CRC组),26例体检健康者作为健康对照组。对两组血清样本γ干扰素(IFN-γ),肿瘤坏死因子-α(TNF-α),白细胞介素-10(IL-10),IL-6,IL-4和IL-2等细胞因子及CEA检测,比较两组差异,分析其与结直肠癌临床病理特征之间的相关性,并评价其单独和联合CEA检测对于结直肠癌的诊断价值。结果CRC组TNF-α,IL-10和IL-6水平高于健康对照组,IL-2浓度低于健康对照组,差异有统计学意义(P<0.05);在细胞因子与临床病理特征相关性分析中,IL-6和IL-4与疾病淋巴结转移、分期和分化有相关性,差异有统计学意义(P<0.05),而IFN-γ,TNF-α,IL-10和IL-2与临床病理特征之间均没有相关性,差异无统计学意义(P>0.05);受试者工作特征曲线(ROC)分析显示CEA曲线下面积(AUC)为0.739,灵敏度和特异度为47.5%和96.2%,曲线下面积(AUC)为0.906,灵敏度和特异度分别为92.3%和79.2%,约登指数为0.715,与单独CEA检测相比,联合检测可提高对结直肠癌的诊断价值。结论CEA联合细胞因子后可提高结直肠癌筛查的灵敏度,TNF-α,IL-10,IL-6和IL-2等炎症因子对结直肠癌诊断及部分临床病理特征具有一定的辅助价值,与CEA联合检测可以为临床结直肠癌的筛查提供一种潜在的辅助诊断方法。Objective To investigate the expression of serum cytokines in colorectal cancer(CRC)and evaluate the diagnostic value of single and combined CEA for colorectal cancer.Methods One hundred and one cases of colorectal cancer were selected as the case group(CRC group),and 26 cases of healthy physical examination were recruited as the control group.Cytokines(IFN-γ,TNF-α,IL-10,IL-6,IL-4 and IL-2)and CEA were detected in the two groups of serum samples.We compared the differences between the two groups,analyzed the correlation between cytokines and clinicopathological features of colorectal cancer,and evaluated the diagnostic value of colorectal cancer between individual detection and combined detection with CEA.Results The levels of TNF-α,IL-10 and IL-6 in the CRC group were higher than those in the healthy control group,and the IL-2 concentration was lower than that in the healthy control group.The difference was statistically significant(P<0.05).In the correlation analysis between cytokines and clinicopathological features,IL-6 and IL-4 were associated with lymph node metastasis,staging and differentiation of the disease,the differences were statistically significant(P<0.05),while there was no correlation among IFN-γ,TNF-α,IL-10 and IL-2 with clinicopathological features.The difference was not statistically significant(P>0.05).Receiver operating characteristic curve(ROC)analysis showed that the area under the curve(AUC)of CEA was 0.739,the sensitivity and specificity were 47.5%and 96.2%,the area under curve(AUC)was 0.906,the sensitivity and specificity were 92.3%and 79.2%,respectively,and the Youden index was 0.715.Compared with CEA alone,the combined detection could improve the diagnostic value of colorectal cancer.Conclusion CEA combined with cytokines can significantly improve the sensitivity of screening CRC.Inflammatory factors such as TNF-α,IL-10,IL-6 and IL-2 have certain auxiliary value for the diagnosis and some clinicopathological features of colorectal cancer.Combined detection with CEA,it may provid
