摘　　要：目的探讨血清胱抑素C(Cys-C)、β2微球蛋白(β2-M)及轻链类型检测在多发性骨髓瘤患者肾脏损害诊断中的应用。方法选取2017年1月至2019年8月在青岛市市立医院确诊的多发性骨髓瘤患者70例作为研究对象,通过检测多发性骨髓瘤患者血清/血浆中各项指标水平,探讨其在多发性骨髓瘤肾损害中改变及意义。结果两组患者Cys-C、β2-M、κ链水平比较,差异有统计学意义(P<0.05)。两组患者尿本周蛋白、轻链类型λ链、κ链例数比较,差异有统计学意义(P<0.05)。尿本周蛋白阳性患者(OR=4.140,95%CI:1.219~14.060)、λ链患者(OR=4.945,95%CI:1.749~13.980)更易发生肾损害。λ链患者Cr、BUN、Cys-C、β2-M水平与κ链患者比较,差异有统计学意义(P<0.05)。结论监测血清Cys-C、β2-M、轻链类型对预测多发性骨髓瘤患者肾损害具有重要临床意义。Objective To investigate the application of serum cystatin C(Cys-C),β2 microglobulin(β2-M)and light chain type detection in the diagnosis of multiple myeloma patients with renal damage.Methods From January 2017 to August 2019,70 patients with multiple myeloma diagnosed in Qingdao municipal hospital were enrolled.By detecting the levels of various indicators in serum/plasma of multiple myeloma patients,the changes and significance of these indicators in renal damage of multiple myeloma patients were discussed.Results The levels of Cys-C,β2-M andκchain were compared between the two groups,the differences were statistically significant(P<0.05).Comparison of bence-jones protein in the urine,light chain typeλchain andκchain cases between the two groups showed statistically significant differences(P<0.05).Patients with positive bence-jones protein in the urine(OR=4.140,95%CI:1.219—14.060)andλchain patients(OR=4.945,95%CI:1.749—13.980)were more likely to develop renal damage.The levels of Cr,BUN,Cys-C andβ2-M inλchain patients were compared with those inκchain patients,the differences were statistically significant(P<0.05).Conclusion Monitoring serum Cys-C,β2-M and light chain type has important clinical significance for predicting renal damage in patients with multiple myeloma.
