改良上睑提肌缩短术与改良CFS悬吊术治疗先天性中重度上睑下垂的效果对比

摘　　要：目的对比改良上睑提肌缩短术与改良联合筋膜鞘(CFS)悬吊术对先天性中重度上睑下垂的治疗效果。方法回顾性分析89例先天性中重度上睑下垂病例(125只患眼)的临床资料,其中有43例(60只患眼)接受改良上睑提肌缩短术,记作A组;余46例(65只患眼)接受改良CFS悬吊术,记作B组。对比术后3个月的矫正效果,不同时刻上睑回退和活动范围,术后3个月内的并发症发生率。结果2组不同病情患眼及总患眼术后3个月矫正效果分布对比差异均无统计学意义(P>0.05),且2组适矫率差异均无统计学意义(P>0.05);2组上睑回退均逐渐减少,活动范围均逐渐增大,本组内差异均有统计学意义(P<0.05),不同时刻2组间对比差异均无统计学意义(P>0.05);B组并发症发生率为2.17%,低于A组的16.28%(P<0.05)。结论改良上睑提肌缩短术与改良CFS悬吊术对先天性中重度上睑下垂的矫治效果相当,但后者安全性更高。Objective To compare the effect of modified levator palpebrae muscle shortening and modified combined fascial sheath(CFS)suspension in the treatment of congenital moderate and severe ptosis.Methods The clinical data of 89 cases(125 eyes)with congenital moderate to severe ptosis were analyzed retrospectively,and among them,43 cases(60 eyes)underwent modified levator palpebrae muscle shortening,which were recorded as group A,and the other 46 cases(65 eyes)were treated with modified CFS suspension as group B.The correction effect,upper eyelid regression and range of motion at different time,and the incidence of complications within 3 months after operation were compared.Results There was no significant difference in the distribution of correction effect between the two groups in different degrees and total eyes at 3 months after operation(P>0.05),the same as the proper correction rates(P>0.05).The upper eyelid regression was gradually reduced and the range of motion was gradually increased in both groups(P<0.05),and there was no significant difference between the two groups at different time points(P>0.05).The incidence of complications in group B was 2.17%,which was lower than that in group A(16.28%)(P<0.05).Conclusion The modified levator palpebrae muscle shortening surgery and modified CFS suspension surgery have the same effect on congenital moderate and severe ptosis,but the latter is more safe.
