摘  要:目的探讨细胞凋亡及Bcl-2蛋白在胃癌发生中的作用及价值。方法选择2018年4月—2019年10月胃癌患者67例作为对象,所有患者均给予手术治疗过程中取病灶组织及癌旁组织(距离病灶≥3 cm)。采用原位末端标记法(TUNEL法)测定不同组织中细胞凋亡情况;采用免疫组织化学法测定不同组织中Bcl-2蛋白阳性率;查阅病理资料,记录胃癌患者性别、年龄、病灶直径、分化类型、淋巴结转移,分析细胞凋亡、Bcl-2蛋白在不同病理中的表达。结果胃癌病灶组织中细胞凋亡率(6.89±0.73)%低于癌旁组织(16.92±2.18)%,差异有统计学意义(t=35.710,P<0.001);病灶组织中Bcl-2蛋白阳性率为62.69%高于癌旁组织8.96%,差异有统计学意义(χ2=42.070,P<0.001);免疫组化结果表明:胃癌病灶组织中Bcl-2蛋白边缘染色明显加深,呈弥漫性分布,而癌旁组织Bcl-2蛋白边缘染色较轻或不表达。结论胃癌的发生、发展与Bcl-2蛋白高表达有关,能抑制细胞凋亡,加剧病情发展,有望成为胃癌新的治疗靶点。Objective To investigate the role and value of Bcl-2 protein in the pathogenesis of gastric cancer.Methods 67 patients with gastric cancer from April 2018 to October 2019 were selected as the subjects.All the patients received surgical treatment,and the lesion tissues and paracancer tissues(distance≥3 cm from the lesion)were taken during the operation.Cell apoptosis in different tissues was determined by TUNEL method.Immunohistochemistry was used to determine the positive rate of Bcl-2 protein in different tissues.Pathological data were consulted to record the gender,age,lesion diameter,differentiation type and lymph node metastasis of gastric cancer patients,and the expression of cell apoptosis and Bcl-2 protein in different pathological conditions were analyzed.Results The apoptosis rate in gastric cancer tissues(6.89±0.73)%was lower than that in adjacent tissues(16.92±2.18)%,the difference was statistically significant(t=35.710,P<0.001);the positive rate of Bcl-2 protein in lesion tissues was 62.69%higher than that in cancer 8.96%of adjacent tissues,the difference was statistically significant(χ2=42.070,P<0.001);immunohistochemical results showed that the marginal staining of Bcl-2 protein in gastric cancer tissues was obviously deepened and showed a diffuse distribution,while the marginal staining of Bcl-2 protein in tissues adjacent to cancer was light or not expressed.Conclusion The occurrence and development of gastric cancer are related to the high expression of Bcl-2 protein,which can inhibit cell apoptosis and aggravate the development of the disease,and is expected to become a new therapeutic target for gastric cancer.
