毛细管超速离心法在地中海贫血患者Rh血型鉴定中的运用

摘　　要：目的探讨毛细管超速离心法在地中海贫血患者鉴定Rh血型的应用价值。方法对本院49名反复输血的重型地中海贫血患者血样本分别进行直接测定Rh血型和经毛细管超速离心法分离自身红细胞并鉴定Rh血型。对比前后检测结果,分析毛细管超速离心法在地中海贫血患者中分离自身红细胞的运用价值。结果反复输血的重度地中海贫血患者经毛细管超速离心技术分离鉴定Rh血型的成功率与输血间隔期呈正相关(r=0.844,P<0.01)。63.26%(31/49)重型地中海贫血患者可以经过毛细管超速离心法分离自身红细胞并成功鉴定Rh血型。经分离鉴定成功的Rh分型结果比例分布为CCDee>CcDEe>ccDEE>CCDEE。结论毛细管超速离心分离自身红细胞有助于反复输血的地中海贫血患者Rh血型的鉴定,为其Rh配型输血,减少同型免疫反应提供基础。Objective To investigate the application value of capillary ultra-centrifugation in the identification of Rh blood group in thalassemia patients.Methods Direct determination of Rh blood type and separation of autoerythrocytes by capillary hypervelocity centrifugation were performed in 49 severe thalassaemia patients who underwent repeated transfusions in our hospital.The results were compared to analyzed the value of capillary ultra-centrifugation in the separation of red blood cells in thalassemia patients.Results The success rate of Rh blood group identification by capillary ultracentrifugation in severe thalassaemia patients with repeated transfusions is positively correlated with the transfusion interva(l r=0.844,P<0.01).63.26%(31/49)of severe thalassemia patients were accurately identified their Rh blood type after the separation of autoerythrocytes by capillary hypervelocity centrifugation.The proportion distribution of Rh typing results after successful separation and identification was CCDee>CcDEe>ccDEE>CCDEE.Conclusion The separation of autoerythrocytes by capillary hypervelocity centrifugation was helpful for the identification of Rh blood type in thalassemia patients with repeated transfusion,providing the basis for their Rh matching transfution and reducing homotypic immune response.
