摘　　要：目的探讨血浆脂蛋白相关磷脂酶(LP-PL)A2、血清铁蛋白(SF)与急性动脉粥样硬化性脑梗死(AACI)的关系。方法128例AACI患者分为两组,即含有高危因素AACI组和不含高危因素AACI组。选择同期健康体检者60例作为对照组。采用磁微粒化学发光法、电化学发光免疫法分别测定各组LP-PLA2、SF水平。结果与对照组比较,含有高危因素AACI组和不含高危因素AACI组LP-PLA2、SF水平均有明显升高(P<0.05)。经相关分析提示,含有高危因素AACI组和不含高危因素AACI组LP-PLA2和SF均无相关性(P>0.05)。经Logistic回归分析提示,LP-PLA2和SF与AACI独立相关(OR=1.881、1.728,P<0.05)。含有高危因素AACI组和不含高危因素AACI组LP-PLA2和SF两者同时检测的阳性率全部高于单项检测,差异均具有统计学意义(χ^2=4.197、4.443,均P<0.05)。结论LP-PLA2和SF可以作为AACI发生风险的评估指标,两者同时检测对AACI发生的预测有一定参考价值。Objective To investigate the relationships between plasma lipoprotein associated phospholipase(LP-PL)A2 and serum ferritin(SF)with acute atherosclerotic cerebral infarction(AACI).Methods 128 patients with AACI were divided into AACI group with high risk factors and AACI group without high risk factors.60 cases of healthy physical examination were selected into control group.The levels of LP-PLA2 and SF in each group were determined by magnetic particle chemiluminescence and electrochemmiluminescent immunoassay.Results The levels of LP-PLA2 and SF were significantly higher in both AACI groups than those of control group(P<0.05).The correlation analysis showed that there was no correlation between LP-PLA2 and SF in both AACI groups(r=0.052,0.064,P>0.05).The logistic regression analysis indicated that LP-PLA2 and SF were independently correlated with AACI(OR=1.881,1.728,P<0.05).The positive rate of the combination of LP-PLA2 and SF in both AACI groups were higher than those of the single test,the differences were statistically significant(χ^2=4.197,P=0.040;χ^2=4.443,P=0.035).Conclusions LP-PLA2 and SF could be used as risk assessment indicators of AACI,and the simultaneous detection of both of them could be used as reference for predicting the occurrence of AACI.
