托吡酯大剂量冲击疗法治疗小儿癫痫的临床效果及安全性

摘　　要：目的研究托吡酯大剂量冲击疗法治疗小儿癫痫的临床效果和安全性。方法84例小儿癫痫患儿,随机分为观察组和对照组,每组42例。两组患儿均采用托吡酯治疗,观察组通过大剂量冲击疗法治疗,对照组通过小剂量用药。比较两组患儿的治疗效果、不良反应发生率及治疗前后言语智商(VIQ)评分和总智商(FIQ)评分。结果观察组患儿治疗总有效率为95.24%,高于对照组的76.19%,差异有统计学意义(P<0.05)。两组患儿不良反应发生率比较,差异无统计学意义(P>0.05)。治疗后,两组患儿VIQ评分和FIQ评分均高于治疗前,且观察组VIQ评分(116.32±2.13)分和FIQ评分(96.86±2.18)分均高于对照组的(110.13±2.12)、(92.41±1.09)分,差异有统计学意义(P<0.05)。结论小儿癫痫采用托吡酯大剂量冲击疗法能够在不产生严重不良反应的前提下提升整体治疗效果,缓解患儿临床症状,是一种理想的治疗手段。Objective To study the clinical effect and safety of pulse therapy of high-dose topiramate in the treatment of epilepsy in children.Methods A total of 84 children with epilepsy were randomly divided into observation group and control group,with 42 cases in each group.Both groups were treated with topiramate,the observation group was treated with high-dose pulse therapy,and the control group was treated with low-dose pulse therapy.The therapeutic effect,incidence of adverse reactions,verbal intelligence quotient(VIQ)score and full intelligence quotient(FIQ)score before and after treatment were compared between the two groups.Results The total treatment effective rate of the observation group was 95.24%,which was higher than 76.19%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05).After treatment,the VIQ score and FIQ score of the two groups were higher than those before treatment,and VIQ score(116.32±2.13)points and FIQ score(96.86±2.18)points of the observation group were higher than(110.13±2.12)and(92.41±1.09)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Pulse therapy of high-dose topiramate for epilepsy in children can improve the overall therapeutic effect and relieve the clinical symptoms of children without serious adverse reactions.It is an ideal treatment method.
