摘 要:目的探讨香菇多糖对中央型非小细胞肺癌肺气亏虚型患者外周血血清及肺泡灌洗液(Bronchoalveolar lavage fluid,BALF)白介素(Interleukin,IL)-4、IL-12的影响。方法以62例新发现不适合手术切除方案的中央型晚期非小细胞肺癌患者为研究对象,随机分为对照组和观察组各31例,对照组采用常规TP化疗方案,观察组采用TP+香菇多糖治疗方案,2组患者治疗前、治疗21 d后均留取外周血及BALF,采用酶联免疫吸附法(Enzyme-linked immunoassay,ELISA)检测IL-4、IL-12的浓度,并对观察组血清与BALF中二者浓度行相关性分析。结果2组患者治疗前外周血清和BALF中IL-4、IL-12浓度差异无统计学意义(P>0.05);与治疗前比,2组患者治疗后血清和BALF中IL-4浓度降低、IL-12浓度提高,差异有统计学意义(P<0.05)。观察组患者治疗前后血清与BALF中IL-4、IL-12表达分别成正相关,差异有统计学意义(P<0.05)。结论中央型非小细胞肺癌肺气亏虚型患者IL-4、IL-12在BALF的表达与在血清中的表达成正相关,香菇多糖可降低外周血及BALF中IL-4、增加IL-12表达,改善患者免疫失衡。Objective To investigate the expression of IL-4 and IL-12 in peripheral blood and BALF of patients of lung qi deficiency with central non-small cell lung cancer. Methods 62 patients of central non-small cell lung cancer,not suitable for surgical treatment,were randomly divided into 2 groups. The control group( 31 cases) used conventional chemotherapy regimens TP,and the observation group( 31 cases) was treated by TP and lentinan. In both groups,peripheral blood was collected before treatment and after 21 days. Enzyme-linked immunoassay( ELISA) was used to detect the concentrations of IL-4 and IL-12,and the correlation between serum and BALF was analyzed.Results There was no significant difference between the two groups of the concentration of IL-4 and IL-12 in peripheral blood serum and BALF before treatment( P > 0. 05). Compared with the control group,the concentration of IL-4 in serum and BALF decreased,while IL-12 increased,and the differences were statistically significant( P < 0. 05). In observation group,before and after treatment,the concentrations of IL-4 in serum and BALF were positively correlated,and the same to IL-12( P < 0. 05). Conclusion The expressions of IL-4 and IL-12 in BALF of patients of lung qi deficiency with central non-small cell lung cancer are positively correlated with those expressions in serum. Lentinan can reduce the expression of IL-4,increase the expression of IL-12 in peripheral blood and BALF,and improve the immune of patients.
