血管内皮生长因子和脂蛋白相关磷脂酶A2在脑梗死合并2型糖尿病患者中的表达及意义

摘　　要：目的:探讨外周血血清血管内皮生长因子(VEGF)和脂蛋白相关磷脂酶A2(Lp-PLA2)在合并有2型糖尿病(T2DM)的脑梗死患者中的表达及临床意义。方法:选取脑梗死患者145例,其中伴T2DM者为研究组(79例),无T2DM者为参照组(66例)。选择同期因高血压、糖尿病或短暂性脑缺血发作就诊患者35例为对照组。入院12 h内均采集静脉血进行VEGF、Lp-PLA2水平的定量检测。结果:研究组血清VEGF、Lp-PLA2水平高于对照组,低于参照组,三组间两两比较差异有统计学意义(P<0.05)。ROC曲线分析显示,参照组血清VEGF和Lp-PLA2的曲线下面积分别为0.761、0.796,研究组的曲线下面积分别为0.571、0.663。结论:伴有T2DM脑梗死患者VEGF和Lp-PLA2表达水平的异常变化可能导致了T2DM患者发生脑梗死后病情重、预后差。Objective:To investigate the expression and clinical significance of vascular endothelial growth factor(VEGF)and lipoprotein associated phospholipase A2(Lp-PLA2)in cerebral infarction patients with type 2 diabetes mellitus(T2DM).Methods:145 patients with cerebral infarction were selected,of which 79 patients with T2DM were selected as study group,and 66 patients without T2DM were selected as reference group.35 patients with hypertension,diabetes mellitus or transient ischemic attack were selected as control group during the same period.Venous blood samples were collected within 12 hours after admission for quantitative detection of VEGF and Lp-PLA2 levels.Results:The levels of VEGF and Lp-PLA2 in the study group were higher than those in the control group and lower than those in the reference group.There was significant difference between the three groups(P<0.05).ROC curve analysis showed that the areas under the curve of serum VEGF and Lp-PLA2 in the reference group were 0.761 and 0.796 respectively,and those in the study group were 0.571 and 0.663 respectively.Conclusion:The abnormal changes of VEGF and Lp-PLA2 expression levels may lead to severe illness and poor prognosis in patients with T2DM after cerebral infarction.
