摘 要:目的探讨电磁导航系统引导神经内镜经单鼻孔蝶窦入路行垂体瘤切除术的应用效果。方法选取在电磁导航系统引导神经内镜下经单鼻孔蝶窦入路行垂体瘤手术的32例患者,术前均行MRI薄层扫描检查,将图像资料输入StealthStation S7神经导航系统中进行三维重建,据此对肿瘤及重要结构做标记,通过电磁导航系统的引导在内镜下辨别蝶窦前壁、颈内动脉、鞍底、海绵窦、斜坡等结构,并切除肿瘤。观察患者术后疗效。结果32例患者经过电磁导航引导内镜经单鼻孔蝶窦入路均顺利完成手术,其中肿瘤全切除25例,次全切除4例,部分切除3例。术后32例患者症状均有不同程度的改善,无严重并发症发生。结论电磁导航引导神经内镜经单鼻孔蝶窦入路施行垂体瘤切除术可提高手术操作的准确性,并发症少,安全有效。Objective To investigate the application effect of electromagnetic navigation guiding neuroendoscopy in the approach of transsphenoidal via single nostril in the performance of pituitary tumor resection.Methods A total of 32 patients undergoing electromagnetic navigation-guided neuroendoscopy in the approach of transsphenoidal via single nostril to perform pituitary tumor resection were selected,and they were given preoperative MRI thin-layer scanning.Three-dimensional reconstruction was performed by in-typing image data into the StealthStation S7 neural navigation system,based on which tumors and important structures were marked.Structures such as the anterior wall of sphenoid sinus,internal carotid artery,sellar floor,cavernous sinus,and clivus were identified via endoscopy guided by the electromagnetic navigation system,and the tumors were removed.The postoperative effects of the patients were observed.Results All the 32 patients had completed the surgery successfully,with electromagnetic navigation-guided endoscopy in the approach of transsphenoidal via the single nostril,of which 25 patients underwent total tumor resection,4 patients with subtotal resection,and 3 patients with partial resection.The symptoms of the 32 patients were improved to varying degrees after operation,without the occurrence of serious complications.Conclusion Electromagnetic navigation-guided neuroendoscopy via transsphenoidal single nostril approach in performing pituitary tumor resection can improve the accuracy of surgical operation,with fewer complications,and it is safe and effective.
