摘 要:目的:比较在胸腹腔镜联合辅助食管癌根治术中应用双肺通气联合CO2气胸与单肺通气两种通气方案对患者肺氧合功能的影响。方法:选择45例ASAⅡ~Ⅲ级拟行全腔镜辅助食管癌根治术患者,分为双肺通气联合CO2气胸组(DLV组)和单肺通气组(OLV组),记录胸腔操作前双肺通气时(T1)、胸腔操作开始后10min(T2)、30min(T3)、60min(T4)、90min(T5)、胸腔操作结束后10min(T1)的肺氧合指标:SpO2、SaO2、PaO2、OI、A-aDO2、RI。结果:DLV组的麻醉插管时间显著性短于OLV组(P<0.05);术中(T2~5)DLV组的肺部氧合功能高于OLV组:SpO2、SaO2、PaO2、OI明显较高且A-aDO2、RI明显较低(P<0.05)。结论:全腔镜辅助食管癌根治术中双肺通气联合CO2气胸通气与单肺通气比较,术中肺部氧合更好且麻醉插管时间更短。Objective: To compare the effects of double lung ventilation combined with CO2 pneumothorax and single lung ventilation on pulmonary oxygenation function in patients undergoing thoracic laparoscopy assisted radical resection of esophageal cancer. Methods: 45 patients with ASAⅡ~Ⅲ grade were selected to undergo total endoscopic assisted radical resection of esophageal carcinoma, and were divided into two groups: double-lung ventilation combined with CO2 pneumothorax group(DLV group) and single-lung ventilation group(OLV group). The pulmonary oxygenation indexes of SpO2, SaO2, PaO2, OI, A-ADO2 and RI at bilateral lung ventilation(T1) before thoracic operation, 10 min(T2), 30 min(T3), 60 min(T4), 90 min(T5) after thoracic operation and 10 min(T1) after thoracic operation were recorded. Results:The anesthesia intubation time in DLV group was significantly shorter than that in OLV group(P<0.05). DLV group had higher Intraoperative(T2-5) pulmonary oxygenation than OLV group: SpO2, SaO2, PaO2 and OI were significantly higher and A-ADO2 and RI were significantly lower(P<0.05). Conclusion:Compared with single lung ventilation, double lung ventilation combined with CO2 pneumothorax ventilation during total endoscopic assisted radical resection of esophageal carcinoma has better intraoperative pulmonary oxygenation and shorter anesthesia intubation time.
