腹腔镜与开放耻骨后前列腺癌根治术治疗前列腺癌的临床疗效及预后比较

摘　　要：目的比较腹腔镜与开放耻骨后前列腺癌根治术治疗前列腺癌的临床疗效及预后。方法选取2017年2月至2018年6月本院收治的60例前列腺癌患者,按随机数字表法分为两组,各30例。对照组接受开放耻骨后前列腺癌根治术治疗,观察组采用腹腔镜前列腺癌根治术治疗。比较两组围术期指标、国际前列腺症状评分(IPSS)、并发症及生化复发率。结果观察组术中出血量为(313.06±53.49)mL、并发症发生率为6.67%,低于对照组的(389.87±76.25)mL、26.67%,住院时间为(14.36±4.07)d,短于对照组的(21.54±6.20)d,手术时间为(276.32±31.08)min,长于对照组的(201.63±26.84)min,差异有统计学意义(P<0.05);两组IPSS评分、生化复发率比较差异无统计学意义。结论前列腺癌患者接受腹腔镜前列腺癌根治术治疗损伤小且并发症少,术后恢复更快,可取得理想的近期、远期治疗效果。Objective To compare the clinical efficacy and prognosis of laparoscopic and open retropubic prostatectomy for prostate cancer.Methods 60 patients with prostate cancer admitted to our hospital from February 2017 to June 2018 were divided into two groups according to the random number table method,with 30 cases in each group.The control group received open radical vaginal prostatectomy and the observation group was treated with laparoscopic radical prostatectomy.The perioperative indicators,international prostate symptom scores(IPSS),complications,and biochemical recurrence rates were compared between the two groups.Results In the observation group,the intraoperative blood loss was(313.06±53.49)mL and the complication rate was 6.67%,which was lower than(389.87±76.25)mL,26.67%of the control group,and the hospitalization time(14.36±4.07)d,which was shorter than the control group(21.54±6.20)d,operation time(276.32±31.08)min,longer than the control group(201.63±26.84)min,with significant difference(P<0.05).There was no significant difference in the IPSS score,biochemical recurrence rate between the two groups.Conclusion Patients with prostate cancer undergoing laparoscopic radical prostatectomy have less damage and fewer complications,and have faster recovery after surgery,which can achieve ideal short-term and long-term treatment effects.
