解毒疏肝汤治疗原发性肝癌介入术后综合症的疗效及对炎症反应、生活质量的影响

摘　　要：目的:观察中药解毒疏肝汤治疗原发性肝癌介入术后综合症的疗效。方法:选择我院2017年1月~2019年10月收治的原发性肝癌肝动脉化疗栓塞术(TACE)后综合症患者108例,按照随机数字表法分为2组,每组54例,对照组常规进行水化、保肝、止吐药物应用等治疗;观察组在对照组治疗基础上应用中药解毒疏肝汤口服,疗程均为2周。比较两组治疗前后主要症状发热、恶心呕吐、肝区疼痛、腹胀、黄疸持续时间;比较两组治疗前后肝功能、炎症因子水平及欧洲癌症研究与治疗组织生命质量测定量表-C30(EORTC QLQ-C30)评分,并比较两组治疗总有效率。结果:观察组术后主要症状发热、恶心呕吐、肝区疼痛、腹胀、黄疸持续时间均短于对照组(P<0.01);丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、直接胆红素(DBIL)、总胆红素(TBIL)、白细胞介素(IL)-1、IL-6和肿瘤坏死因子-α(TNF-α)均低于对照组(P<0.01),EORTC QLQ-C30量表功能领域、总体健康状况评分高于对照组(P<0.01);症状领域、单测项目评分低于对照组(P<0.01),治疗总有效率高于对照组(P<0.05),差异均有统计学意义。结论:解毒疏肝汤治疗TACE术后栓塞综合症有利于缓解症状,改善肝功能,提高生活质量,效果显著,其机制可能与改善机体的炎症反应有关。Objective:To investigate the therapeutic effect of Jiedu Shugan Decoction on the syndrome of primary liver cancer after interventional operation.Methods:108 patients with post TACE syndrome of primary liver cancer from January 2017 to October 2019 in our hospital were selected and divided into two groups according to the random number table method,with 54 cases in each group.The controlled group were treated with hydration,liver protection and antiemetic drugs.The observation group were treated with Jiedu Shugan Decoction on the basis of the treatment of the controlled group,and the treatment course was for 2 weeks.The duration of fever,nausea and vomiting,pain in liver area,abdominal distention and jaundice were compared before and after treatment of the two groups.The liver function,the level of inflammatory factors and the score of European cancer research and treatment tissue quality of life(EORTC QLQ-C30)were compared before and after treatment of the two groups,and the total effective rate of treatment of the two groups was compared.Results:The duration of fever,nausea and vomiting,liver pain,abdominal distention and jaundice of the observation group were shorter than that of the controlled group(P<0.01).ALT,AST,DBIL,TBIL,IL-1,IL-6 and TNF-αof the observation group were lower than those of the controlled group(P<0.01).The score of EORTC QLQ-C30 was higher than that of the controlled group(P<0.01),the score of symptom area and single test item was lower than that of the controlled group(P<0.01),and the total effective rate of treatment was higher than that of the controlled group(P<0.05).Conclusion:Jiedu Shugan Decoction is helpful to relieve the symptoms,improve the liver function,improve the quality of life,and its mechanism may be related to the improvement of inflammatory response.
