摘  要:目的原发性眼睑痉挛(primary blepharospasm,BSP)患者不仅存在局部解剖脑区功能活动的异常,也存在脑区间功能连接的异常,但是否存在脑功能网络连接的改变仍不清楚。本研究拟采用基于体素的中心度(degree centrality,DC)分析方法,观察BSP患者全脑功能网络连接的改变,并分析这种改变与眼睑痉挛严重程度的关系。方法纳入BSP患者42例及健康对照42例。采用静息态功能磁共振数据并基于体素DC的全脑分析方法观察BSP患者与健康对照DC值的差异;采用Spearman偏相关分析方法(校正年龄、性别、肉毒毒素治疗时程)分析功能连接存在差异脑区DC值与患者病程、眼睑痉挛严重程度的相关性。结果与健康对照组比较,BSP患者右侧上顶叶DC值显著增加(P<0.01,AlphaSim校正)。Spearman偏相关分析结果显示BSP患者右侧上顶叶DC值与病程、眼睑痉挛严重程度均无显著相关性。结论BSP患者存在右侧上顶叶相关功能网络连接的改变,但这种改变与疾病严重程度并无关系。Objective Abnormal spontaneous neural activity and functional connectivity in multiple brain regions were reported in patients with primary blepharospasm(BSP).However,the changes of whole-brain network in BSP remain unknown.We aim to investigate the changes of the whole-brain network of BSP and whether these changes were related to disease features using voxel-wise degree centrality(DC)analysis.Methods Forty-two patients with BSP and 42 healthy controls were recruited.The voxel-wise whole-brain DC analysis based on functional magnetic resonance imaging was used to compare intergroup whole-brain network differences.Spearman partial correlation analyses were performed to assess associations between DC value of significantly difference areas and duration and disease severity in BSP patients after adjusting for age,duration of disease,and duration of BoNT treatment as covariates.Results Compared with healthy controls,BSP patients showed significantly increased DC value in the right superior parietal lobule(P<0.05,Alphasim corrected).No significantly corrections were found between DC value of right superior parietal lobule and duration and Jankovic rating scale in BSP patients.Conclusion These results indicated that BSP patients may have abnormal whole-brain network change.The change has no relation with duration or disease severity.
