瞬目反射联合面神经传导在早期特发性面神经炎中的应用

摘　　要：目的探究瞬目反射联合面神经传导检测在早期特发性面神经炎中的临床应用效果。方法选择2018年3月~2019年11月在我院门诊就诊的100例早期特发性面神经炎患者进行研究,均进行瞬目反射、面神经传导检测,分析其检测结果与患者临床表现、痊愈时间的关系。结果(1)在瞬目反射检测中,100例患者患侧R1、R2、R2'波未出波百分比明显高于健侧,且患侧各波的潜伏期显著长于健侧(P<0.05)。瞬目反射检测全出波、1个未出波、2个未出波、3个未出波患者之间的痊愈时间比较,差异无统计学意义(P>0.05)。(2)面神经传导检测显示患侧的口轮匝肌、眼轮匝肌M波潜伏期显著长于健侧(P<0.05)。M波下降幅度<30%、30%~50%、>50%之间的痊愈时间比较,差异有统计学意义(P<0.05),且经Pearson相关性分析发现,M波下降幅度与患者痊愈时间呈正相关(P<0.05)。结论瞬目反射检测对特发性面神经炎有定位诊断作用,而面神经传导检测可直接反映面神经损害程度,两种检测方法联合产生协同互补作用,对于特发性面神经炎的早期诊断、病情严重程度、预后判断具有较高的参考价值。Objective To explore the clinical application effect of blink reflex(BR)test combined with facial nerve conduction(FNC)test on early idiopathic facial neuritis.Methods A total of 100 patients with early idiopathic facial neuritis treated in the outpatient service of our hospital from March 2018 to November 2019 were selected for study,and all of them were given BR and FNC tests.The relationships between the results of the tests and the patients'clinical manifestations and recovery time were analyzed.Results(1)In the BR test,among the 100 patients,the absent wave rate of R1,R2 and R2'waves on the affected side was obviously higher than that on the healthy side,and the latency period of each wave on the affected side was significantly longer than that on the healthy side(P<0.05).There was no statistically significant difference in recovery time among the patients tested with full waves,1 absent wave,2 absent waves and 3 absent waves(P>0.05).(2)The FNC test showed that the latency period of M wave in orbicularis oris muscle and orbicularis oculi muscle on the affected side was significantly longer than that on the healthy side(P<0.05).There were statistically significant differences in recovery time among the cases of M wave decline amplitude being<30%,between 30%and 50%,and>50%(P<0.05),and Pearson correlation analysis showed that M wave decline amplitude was positively correlated with the recovery time of the patients(P<0.05).Conclusion BR test can be used for determining the diagnosis of idiopathic facial neuritis,while FNC test can directly reflect the degree of facial nerve damage.The combination of the two tests can produce synergistic and complementary effect.It has high reference value for early diagnosis,severity and prognosis judgment of idiopathic facial neuritis.
