18F-氟脱氧葡萄糖PET/CT最大标准化摄取值与肺癌临床相关因素的关系研究

摘　　要：目的探讨18F-氟脱氧葡萄糖(FDG)正电子发射断层扫描(PET)/计算机断层扫描术(CT)最大标准化摄取值(SUVmax)与肺癌临床相关因素的关系。方法选取中国人民解放军联勤保障部队第九二三医院2018年1月至2020年1月收治的肺癌患者137例。治疗前进行18F-FDG PET/CT检查;用感兴趣区(ROI)勾画技术,测量病灶SUVmax。分析SUVmax与相关因素的关系。结果患者病灶SUVmax与肿瘤最大径之间,呈现明显正相关(r=0.416,P<0.05);SUVmax与肿瘤临床分期、病理类型之间,同样呈正相关(r=0.245,t=0.273,P<0.05);SUVmax与肿瘤分化程度呈现明显的负相关(r=-0.652,P<0.05)。Logistic多元线性回归分析显示,SUVmax肿瘤最大径与分化程度对其存在多重共线性。肿瘤分化程度与SUVmax呈现明显的负相关(β=-0.617,P<0.05)。结论SUVmax可以间接反映肿瘤细胞的分化、增殖程度,病理类型及肿瘤临床分期并非SUVmax的明显影响因素。Objective To investigate the relationship between the maximum standardized uptake value(SUVmax)of 18F fluorodeoxyglucose(FDG)positron emission tomography(PET)/computed tomography(CT)and clinical related factors of lung cancer.Methods A total of 137 patients with lung cancer admitted to the 923 Hospital of the Joint Logistics Support Force of the Chinese People's Liberation Army from January 2018 to January 2020 were selected.Before treatment,18F-FDG PET/CT examination was carried out;the SUVmax of the lesions was measured with the delineation technology of ROI;the relationship between SUVmax and related factors was analyzed.Results There was a significant positive correlation between the SUVmax of the patient’s lesion and the maximum diameter of the tumor(r=0.416,P<0.05);the SUVmax was also positively correlated with the clinical stage and pathological type of the tumor(r=0.245,t=0.273,P<0.05);SUVmax showed a significant negative correlation with the degree of tumor differentiation(r=-0.652,P<0.05).Logistic multiple linear regression analysis showed that there is multicollinearity between the maximum diameter and differentiation degree of SUVmax tumor.The degree of tumor differentiation was significantly negatively correlated with SUVmax(β=-0.617,P<0.05).Conclusion SUVmax could indirectly reflect the degree of differentiation and proliferation of tumor cells,and the pathological type and clinical stage of the tumor are not obvious influencing factors of SUVmax.
