运动神经元病证候要素分布及组合规律探析

摘　　要：目的探讨运动神经元病证候要素分布及组合规律。方法检索中国知网、万方数据库、维普数据库全部期刊,共纳入64篇文献,1353例运动神经元病文献病例,对符合要求的运动神经元病中医证候名称进行规范,提取证候要素,对原始数据建立Excel数据库,运用SPSS25.0统计文献中运动神经元病中医证型、证候要素及证候靶位的频次及频率,并运用系统聚类的方法分析运动神经元病证候要素分布及组合的规律。结果经得规范的运动神经元病证候名称43个,提取证候要素14个,频率最高的依次为气虚(64.52%)、血虚(23.06%)、精亏(18.33%);证候靶位8个,频率最高的依次为脾(67.18%)、肾(54.69%)、胃(28.6%)、肝(25.28%)。证候要素组合形式归为三类,单一证候要素最多见,气虚所占比率最高;其次为两证候要素组合,具体组合形式以精亏+血虚出现的频次最高;三种及以上证候要素组合中以精亏+血虚+湿+热与气虚+痰+瘀最为多见。证候要素和证候靶位系统聚类可判别为3类:(1)气虚、脾、肾;(2)血虚、肝、胃;(3)精亏、湿、痰、血瘀、热、阴虚、阳虚、内风、气郁、肺、经络等。结论运动神经元病证候要素以气虚、精亏、血虚等虚证为主,兼有湿、热、痰、瘀等实证,多呈虚实夹杂之象,为进一步研究运动神经元病的中医病机、规范辨证诊断及临床治疗提供参考和依据。Objective To explore the rules of distribution and combination of syndrome elements in motor neuron disease.Methods All the journals of china national knowledge infrastructure(CNKI),Wanfang database,and VIP database were searched,a total of 64 articles were included,with 1353 documented cases of motor neuron disease.The TCM syndrome names of motor neuron disease that meet the requirements were standardized,the syndrome elements were extracted,the Excel database for original data was set up.SPSS 25.0 was used to calculate the frequency and frequency rate of TCM syndrome types,syndrome elements and syndrome targets of motor neuron disease in the literature.And the rules of distribution and combination of syndrome elements in motor neuron disease were analyzed by systematic clustering method.Results 43 standard syndrome names of motor neuron disease were obtained.14 syndrome elements were extracted.The highest frequency is qi deficiency(64.52%),blood deficiency(23.06%)and essence deficiency(18.33%).The syndrome targets was 8,with the highest frequency successively was spleen(67.18%),kidney(54.69%),stomach(28.6%)and liver(25.28%).The combination forms of syndrome elements are classified into three types,the single syndrome element is the most common,the proportion of qi deficiency is the highest;the second is the combination of two syndrome elements,and the specific combination form of‘essence deficiency and blood deficiency’is the most highest frequency;Among combinations of three or more syndrome elements,the most common ones is‘essence deficiency,blood deficiency,dampness,heat,heat and qi deficiency,phlegm,blood stasis’.Syndrome elements and syndrome targets can be classified into three categories by system clustering:(1)qi deficiency,spleen,kidney;(2)blood deficiency,liver,stomach;(3)essence deficiency,dampness,phlegm,blood stasis,heat,yin deficiency,yang deficiency,internal wind,qi stagnation,lung,meridians,etc.Conclusion The main syndrome elements of motor neuron disease are deficiency syndromes,such as
