摘 要:目的探讨生物反馈放松疗法联合艾司唑仑治疗卒中后睡眠障碍患者的临床效果。方法选取2018年1月至2019年12月我院收治的96例卒中后睡眠障碍患者,根据简单随机化法将其分为艾司唑仑组(n=48)和联合组(n=48)。艾司唑仑组予以艾司唑仑治疗,联合组予以生物反馈放松疗法联合艾司唑仑治疗。比较两组的临床疗效及治疗前、后的睡眠多导图参数、匹兹堡睡眠质量指数量表(PSQI)评分、血清5-羟色胺(5-HT)、脑源性神经营养因子(BDNF)水平;比较联合组治疗前、后的额肌肌电均值(AEMG)。结果联合组的治疗总有效率高于艾司唑仑组(P<0.05)。治疗后,联合组的睡眠效率、觉醒次数、睡眠潜伏期、睡眠总时间均优于艾司唑仑组(P<0.05);治疗后,联合组的PSQI评分低于艾司唑仑组,血清5-HT、BDNF水平高于艾司唑仑组(P<0.05)。联合组治疗后的额肌AEMG低于治疗前(P<0.05)。结论生物反馈放松疗法联合艾司唑仑应用于卒中后睡眠障碍患者中的效果显著,能有效放松肌肉,改善患者睡眠质量,其机制可能与血清5-HT、BDNF水平升高有关。Objective To explore the clinical effect of biofeedback relaxation therapy combined with estazolam in the treatment of patients with sleep disorders after stroke.Methods A total of 96 patients with sleep disorders after stroke anmitted in our hospital from January 2018 to December 2019 were selected and divided into estazolam group(n=48)and combined group(n=48)by simple randomization method.The estazolam group was treated with estazolam,and the combined group was treated with biofeedback relaxation therapy combined with estazolam.The clinical efficacy,sleep polysomnography parameters,Pittsburgh sleep quality index(PSQI)score,serum 5-hydroxytryptamine(5-HT)and brain-derived neurotrophic factor(BDNF)levels before and after treatment were compared between the two groups;the average electromyography(AEMG)of frontal muscle before and after treatment in the combined group was compared.Results The total effective rate of treatment of the combined group was higher than that of the estazolam group(P<0.05).After treatment,the sleep efficiency,number of awakenings,sleep latency and total sleep time of the combined group were better than those of the eszolam group(P<0.05);after treatment,the PSQI score of the combined group was lower than that of the estazolam group,the serum 5-HT and BDNF levels were higher than those of the estazolam group(P<0.05);the AEMG of frontal muscle in the combined group after treatment was lower than that before treatment(P<0.05).Conclusion Biofeedback relaxation therapy combined with estazolam has significant effects applied in patients with sleep disorders after stroke,it can effectively relax muscles and improve sleep quality of patients.The mechanism may be related to elevated serum 5-HT and BDNF levels.
