脑脊液乳酸和脑脊液乳酸脱氢酶在新生儿化脓性脑膜炎中的应用价值

摘　　要：目的:探讨新生儿化脓性脑膜炎患儿脑脊液乳酸(CSF-LA)和脑脊液乳酸脱氢酶(CSF-LD)水平变化及应用价值。方法:选取55例新生儿化脓性脑膜炎患儿为研究组、25例呼吸系统感染患儿为对照组,取治疗前、治疗后的脑脊液(CSF)标本检测CSF-LA、CSF-LD含量,比较其差异。结果:治疗前的CSF-LA、CSF-LD均高于治疗后,并且高于对照组,差异有统计学意义(P<0.05);CSF-LA、CSF-LD特异性为96%、88%,灵敏度为94.55%、100%;研究组CSF-LA与CSF-LD、CRP、TP呈正相关,差异有统计学意义(r=0.694,0.527,0.794;P<0.05),与GLU、CL呈负相关,差异有统计学意义(r=-0.514,-0.433;P<0.05)。结论:CSF-LA、CSF-LD的水平变化与新生儿化脓性脑膜炎患儿病情密切相关,可作为临床诊断、判断病情的有利参考指标。Objective To investigate the changes of cerebrospinal lactic acid(CSF-LA)and cerebrospinal lactate dehydrogenase(CSF-LD)levels and their application value in neonates with suppurative meningitis.Method Fifty-five newborn children with suppurative meningitis were selected as the study group and 25 children with respiratory infection as the reference group.Cerebrospinal fluid(CSF)specimens were taken before and after treatment to detect the contents of CSF-LA and CSF-LD and compare their differences.Results Both CSF-LA and CSF-LD before treatment were higher than those after treatment and higher than those of the reference group,with statistically significant difference(P<0.05).The specificity of CSF-LA and CSF-LD were 96%,88%,sensitivity 94.55%and 100%.In the study group,CSF-LA was positively correlated with CSF-LD,CRP and TP(r=0.694,0.527,0.794,P<0.05),was negatively correlated with GLU and CL(r=-0.514,-0.433,P<0.05).Conclusion The changes of CSF-LA and CSF-LD levels are closely related to the condition of newborn children with suppurative meningitis,which can be used as a useful reference index for clinical diagnosis and judgment of the disease.
