摘 要:目的:探讨骨转移癌核医学治疗效果与钙磷代谢的相关性。方法:选取2018年1月-2019年7月于本院接受99Tc-MDP联合氯化锶(89Sr)治疗的骨转移癌患者30例。观察所有患者骨痛缓解情况、治疗效果及不良反应发生情况。根据治疗效果将患者分为有效组与无效组,比较两组治疗前后的碱性磷酸酶(AKP)、血甲状旁腺素(PTH)、血钙及血磷。分析钙磷代谢情况与治疗效果的相关性。结果:治疗后,30例患者骨痛缓解率为86.67%(26/30),治疗总有效率为90.00%(27/30),不良反应发生率为16.67%(5/30)。第1次治疗后,有效组PTH、AKP、血钙和血磷水平均低于无效组(P<0.05)。第2次治疗后,有效组PTH、AKP、血钙和血磷水平均低于无效组(P<0.05)。PTH、AKP、血钙和血磷水平与治疗效果均呈负相关(P<0.05)。结论:99Tc-MDP联合89Sr治疗骨转移癌患者可有效缓解骨痛,抑制骨转移,不良反应较少,钙磷代谢水平和核医学治疗效果呈负相关。Objective:To explore the correlation between the effect of nuclear medicine on bone metastases cancer and metabolism of calcium and phosphorus.Method:From January 2018 to July 2019,30 patients with bone metastases cancer treated with 99Tc-MDP combined with strontium chloride (89Sr) in our hospital were selected.Bone pain relief,treatment effect and adverse reactions were observed in all patients.According to the treatment effect,the patients were divided into effective group and ineffective group.The alkaline phosphatase (AKP),blood parathyroid hormone (PTH),blood calcium and blood phosphorus were compared between the two groups before and after treatment.The correlation between metabolism of calcium and phosphorus and treatment effect was analyzed.Result:After treatment,the bone pain relief rate of the 30 patients was 86.67% (26/30),the total effective rate was 90.00% (27/30),and the incidence of adverse reactions was 16.67% (5/30).After the first treatment,PTH,AKP,serum calcium and serum phosphorus levels in the effective group were all lower than those in the ineffective group (P<0.05).After the second treatment,PTH,AKP,serum calcium and serum phosphorus levels in the effective group were all lower than those in the ineffective group (P<0.05).PTH,AKP,serum calcium and phosphorus levels were negatively correlated with treatment effect (P<0.05).Conclusion:The combination of 99Tc-MDP and 89Sr in the treatment of patients with bone metastases cancer can effectively relieve bone pain,inhibit bone metastasis,and have fewer adverse reactions,metabolism of calcium and phosphorus level is negatively correlated with the therapeutic effect of nuclear medicine.
