326例胃癌手术标本HER2状态分析

摘　　要：目的:应用2016版胃癌HER2检测指南分析HER2状态及其意义。方法:选取2016-2019年就诊于福建省立医院的胃癌手术病例326例,均进行HER2荧光原位杂交(FISH)双探针法检测,其中152例同时进行HER2免疫组织化学染色(IHC)。采用2016版胃癌HER2检测指南标准对HER2检测结果进行判读分析。结果:按2016版胃癌HER2检测指南标准,HER2 FISH阳性61例(18.71%),异质性7例(2.15%),男女的HER2 FISH检验结果比较,差异无统计学意义(χ^2=1.912,P=0.384)。HER2 IHC蛋白过表达率16.45%,男女ICH检测结果比较,差异有统计学意义(χ^2=11.156,P=0.025);IHC结果为0~1+、3+中,FISH与IHC符合率分别高达97.65%和95.83%;17号染色体多体检出率27.61%,男女性别相比较,差异有统计学意义(χ^2=8.693,P=0.013);17号染色体不同倍体间HER2 FISH及HER2 IHC相比,差异均无统计学意义(χ^2=1.627、1.299,P=0.443、0.861)。结论:HER2 IHC及FISH检测两者有很好的符合率,HER2 IHC检测结果以及17号染色体多体检出率不同性别间有差异,17号染色体倍体与胃癌HER2状态无关。Objective:To explore the influence and significance of the revised 2016 guidelines for HER2 testing in gastric cancer.Method:A total of 326 patients with gastric cancer in Fujian Provincial Hospital from 2016 to 2019 who were selected for the detection of HER2 FISH dual probe method,of which 152 cases were simultaneously subjected to HER2 IHC.The results were reinterpreted according to the 2016 edition guidelines for HER2 detection in gastric cancer.Result:According to the 2016 version of the guidelines for the detection of HER2 in gastric cancer,the number of positive and heterogeneous cases were 61 cases (18.71%) and 7 cases (2.15%) in FISH test,there was no statistically significant difference between male and female (χ^2=1.912,P=0.384).The overexpression rate of protein was 16.45% in IHC test,compared with male and female,the difference was statistically significant (χ^2=11.156,P=0.025).In 0-1+ and 3+,the coincidence rate of FISH and IHC as high as 97.65% and 95.83% respectively.The detection rate of chromosome 17 polysomy was 27.61%,compared with male and female,the difference was statistically significant (χ^2=8.693,P=0.013).IHC and FISH test results showed that the differences in chromosome 17 ploidy cases were not statistically significant (χ^2=1.627,1.299;P=0.443,0.861).Conclusion:There is a good agreement between HER2 IHC and FISH.The results of HER2 IHC and the detection rate of polysomy on chromosome 17 may be related to gender,and the chromosome 17 ploidy may not be related to the detection of HER2 status of gastric cancer.
