胰腺囊性病变诊断中多层螺旋CT的临床应用及影像学征象研究

摘　　要：目的:探究胰腺囊性病变诊断中多层螺旋CT的临床应用及影像学征象。方法:选取2018年1月-2020年2月本院收治的胰腺囊性病变患者54例的临床资料进行回顾性分析,所有患者均进行病理检测和多层螺旋CT检查。以病理诊断为金标准,分析多层螺旋CT对胰腺囊性病变的诊断符合率及影像学征象。结果:多层螺旋CT对胰腺囊性病变的诊断符合率为96.3%(52/54)。34例肿瘤性囊性病变患者中浆液性囊腺瘤(26.5%)发生率最高,其次为实性假乳头状瘤(23.5%)、囊腺癌(14.7%);20例非肿瘤性囊性病变患者中,假囊肿(75.0%)发生率最高,其次为胰腺真囊肿(15.0%)、肠源性囊肿(10.0%)。肿瘤性囊性病变中囊肿多发、位置在胰腺管远端和存在腹壁结节的例数多于非肿瘤性囊性病变(P<0.05)。结论:多层螺旋CT对胰腺囊性病变的诊断符合率较高,能够清楚的显现胰腺囊性病变的性质、胰腺周围形态学变化等具体情况,为术前评估患者情况、制定针对性的治疗方案等提供重要依据,值得推广应用。Objective:To explore the clinical application of multi-slice spiral CT in the diagnosis of pancreatic cystic lesions and imaging signs.Method:The clinical data of 54 patients with cystic lesions of pancreas treated in our hospital from January 2018 to February 2020 were analyzed retrospectively.All patients were examined by pathology and multi-slice spiral CT.The diagnosis coincidence rate and imaging signs of pancreatic cystic lesions with mult-slice spiral CT were analyzed with pathological diagnosis as the gold standard.Result:The diagnostic coincidence rate of mult-slice spiral CT was 96.3% (52/54).Among 34 patients with neoplastic cystic lesions,the incidence of serous cystadenoma (26.5%) was the highest,followed by solid pseudopapilloma (23.5%) and cystadenocarcinoma (14.7%).Among 20 patients with non-neoplastic cystic lesions,the incidence of pseudocyst (75.0%) was the highest,followed by true pancreatic cyst (15.0%) and enterogenic cyst (10.0%).The number of cases with multiple cysts,distal to the pancreatic duct,and abdominal wall nodules in neoplastic cystic lesions were higher than those in non-neoplastic cystic lesions (P<0.05).Conclusion:Multi-slice spiral CT has a high diagnosis coincidence rate in the diagnosis of pancreatic cystic lesions,and can clearly show the nature of pancreatic cystic lesions,pancreatic peripheral morphological changes and other specific conditions,providing an important basis for preoperative evaluation of the patient,the formulation of targeted treatment,and so on.It is worthy of promotion and application.
