摘　　要：目的:探讨婚姻状态与宫颈癌患者远期预后的关系。方法:收集美国国家癌症研究所SEER数据库中2010年至2015年间诊断为宫颈癌的患者临床资料,回顾性分析婚姻状态与肿瘤特异性死亡(CSM)的关系,应用X-tile软件确定年龄及肿瘤大小的最佳截止值,应用GraphPad Prism 8.0和SPSS 23.0统计学软件进行单因素和多因素Cox分析,观察婚姻状况(已婚、单身、丧偶及离异/分居)对宫颈癌患者远期生存预后的影响。结果:共入组7912例宫颈癌患者,其中已婚3676例(46.5%)、单身2566例(32.4%)、丧偶572例(7.2%)、离异/分居1098例(13.9%)。Cox多因素分析结果显示,已婚宫颈癌患者预后生存率优于其他3组,丧偶组患者死亡风险最高(P<0.05)。结论:已婚患者宫颈癌预后最佳,婚姻状况对宫颈癌生存预后有关,能够作为预测宫颈癌患者死亡风险的独立因素。Objective:To investigate the relationship between marital status and long-term prognosis of cervical cancer.Methods:The clinical data of patients diagnosed with cervical cancer were retrieved in National Cancer Institute SEER database from 2010 to 2015.The relationship between marital status and cancer-specific death(CSM)was analyzed retrospectively.The optimal cut-off age and tumor size was determined by X-tile software.GraphPad Prism 8.0 and SPSS version 23.0 statistical software were used for univariate analysis and multivariate Cox regression,so as to investigate the impact of marital status(married,single,widowed and divorced/separated)on the long-term survival and prognosis of cervical cancer.Results:A total of 7912 patients with cervical cancer were enrolled,including 3676 married(46.5%),2566 single(32.4%),572 widowed(7.2%),1098 divorced or separated(13.9%)subjects.Multivariate Cox analysis showed that the survival rate of married subjects was more favorable than the other three groups,and the risk of death was highest in widowed subjects(P<0.05).Conclusion:The prognosis of cervical cancer appears to be most favorable in married patients.Marital status is correlated to the survival and prognosis of cervical cancer,and can be used as an independent predictor for the risk of death in patients with cervical cancer.
