踝部骨膜骨肉瘤的临床病理诊断与鉴别诊断

摘　　要：目的报道1例骨膜骨肉瘤(Periosteal osteosarcoma)的临床病理诊断,回顾其临床表现、组织学特征和鉴别诊断。方法收集1例外院会诊骨膜骨肉瘤病例的临床资料;病理组织学观察;回顾相关文献。结果大体观察:肿瘤表面光滑,有纤维性假包膜,肿瘤略呈分叶状,切面灰蓝色、实性、质硬,有沙砾感;镜下见,肿物主要由透明软骨肉瘤样成分构成;见肿瘤性成骨;可见增生的梭形细胞和多边形肿瘤细胞背景,局灶可见黏液样变性。结论骨膜骨肉瘤为中低度恶性骨表面恶性肿瘤,其特征性的影像学表现、细胞形态、组织学形态及免疫表型,在明确诊断时具有指导意义。Objective To report the pathologic diagnosis about one Periosteal osteosarcoma s case,then reviewed the clinical manifestation,histologic characteristics,and differential diagnosis.Methods Collected the clinical data from one consulted Periosteal osteosarcoma patient,and executed the pathological check,and reviewed the pertinent literature.Results Gross observation:the tumor had a smooth surface,clear border and fibro-pseudocapsule.Its cross section was grey-blue,substantial,hard texture and graveliness.Microscopic Observation:the tumor was composed of lobulated hyaline cartilage,and the neoplastic osteogenesis also appeared,along with moderate to severe atypia and active mitotic activity,and local myxoid degeneration.Conclusion This periosteal Osteosarcoma is one superficial malignant neoplasm of bone with moderate to low grade malignancy.Its characteristic imaging,cell morphology,histological morphology and immunophenotype provided us some guidance in the definite diagnosis.
