摘　　要：目的探讨腰骶部脊髓黏液乳头状型室管膜瘤(MPE)与周围组织粘连的MRI特征及其与手术结果的相关性。方法回顾性分析经病理证实的14例腰骶部脊髓MPE患者(共22个病灶),观察术前MRI特征,结合术后病理结果分析其与手术所见的相关性。结果MR增强T1WI表现为不均匀强化肿瘤较均匀强化者更易与周围组织粘连(P<0.05);T2WI信号表现与病灶和周围组织粘连情况无显著相关性(P>0.05)。22个病灶均经手术完全切除。3例肿瘤术后复发,病灶均与周围组织粘连。结论腰骶部MPE术前增强MRI表现为非均匀强化多提示病灶与周围组织粘连,术后较无粘连者更易复发。Objective To observe MRI characteristics of lumbosacral spinal myxopapillary ependymomas(MPE)adhered with surrounding structures and the correlation with surgical findings.Methods Data of 14 patients with 22 lumbosacral spinal MPE lesions confirmed by pathology were retrospectively analyzed.Pre-operation MRI features,intra-operation findings and post-operative pathological results were observed,and their correlations were also analyzed.Results Tumors showed heterogeneously enhancement on contrast-enhanced MR T1WI were more likely to adhere to surrounding structures than those with homogeneously enhanced(P<0.05),whereas no significant association was found between T2WI characteristics of tumors and adhesion of lesions to surrounding structures(P>0.05).All 22 lesions were gross-total resected through surgical operations.Tumors recurred after first resection in 3 cases and adhered to surrounding structures.Conclusion Lumbosacral spinal MPE with heterogeneous enhancement on pre-operative MRI tended to adhere with surrounding structures,which were more likely to recur after surgical resection than tumors without adhesion.
